Objective:To ensure whether peripheral blood Tenascin-C can as a novel predictor for coronary artery lesion by comparing peripheral blood Tenascin-C level among on admission, after treatment and recovery in children with Kawasaki disease and comparing Tenascin-C level among Kawasaki disease, anaphylactoid purpura, infectious mononucleosis patients and healthy control. Methods:Peripheral blood TN-C level was measured in children with KD,anaphylactoid purpura, infectious mononucleosis patients or healthy during January 1,2018 and November 1,2018.Then statistically analyzed and compared. Results:We collected blood samples of 44 KD patients(CAL~+:21;CAL~-:23),39 anaphylactoid purpura patients,24 infectious mononucleosis patients and 36 healthy controls were enrolled. The study found TN-C levels on admission and after treatment were significantly higher in patients than in patients during convalescence after IVIG administration(32.0±13.8,33.5±11.4,23.3±10.8ng/ml,respectively;P<0.05),and correlated positively with C-reactive protein(P=0.038),and negatively with sodium level(P=0.004).But there was no statistically significant difference in TN-C level between CAL~+and CAL~-(on admission:31.7±15.4 vs 32.3±12.5ng/ml;after IVIG treatment:32.2±11.6vs 34.8±11.3ng/ml;convalescence:22.6±7.3vs24.0±13.4ng/ml;P>0.05,respectively).In addition,TN-C level with KD,.anaphylactoid purpura and infectious mononucleosis were higher than healthy children(32.0±13.8,37.2±18.2,56.6±23.1,24.0±8.05ng/ml,respectively,P<0.05).Conclusion:This study was not sufficient to prove that peripheral blood TN-C can be used as a novel predictor for coronary artery lesion in KD,nor can it be used as a maker of vascular injury, but it may be used as an indicator of immune response in the acute phase of Kawasaki disease. In addition, this study was the first to find that TN-C was increased in infectious mononucleosis.
