Objective: Through the study of the incidence,age,location,level of tumorigenesis,pathological diagnosis and treatment of upper gastrointestinal submucosal tumors(SMT)we can improve the level of understanding and diagnosis and treatment of SMT,so as to achieve early diagnosis and treatment of SMT.Methods: Retrospectively reviewed the cases of SMT diagnosed by digestive endoscopy and pathology department from 2016-8 to 2018-12 and partially treated by operation,a total of 735 cases of SMT diagnosed by endoscopy or endoscopic ultrasonography(EUS)were collected.210 cases were diagnosed by EUS,50 cases were treated by endoscopic minimally invasive surgery and pathological results were obtained.Statistical data of gender,age,location of lesions,level of tumorigenesis,pathological diagnosis and treatment methods were collected.The epidemiological and clinicopathological characteristics of SMT were analyzed by comparing the above-mentioned data.Results: In this study,330 males and 405 females,aged from 7 to 86 years,were found in 228 cases of esophagus,406 cases of stomach and 101 cases of duodenum.Among them,69 cases of esophagus,121 cases of stomach and 20 cases of duodenum were detected by EUS.Endoscopic minimally invasive surgery was performed and pathological results were obtained in 23 cases of gastrointestinal stromal tumors(GIST),14 cases of leiomyoma,6 cases of Heterotopic pancreas(HP),and 7 cases of other pathologies.There was no significant difference between the sexes(P> 0.05).The incidence of SMT was significantly different in different age groups(the highest incidence was 41-60 years old).There was a significant difference in the ratio of SMT sites in the three parts of esophagus,stomach and duodenum(P< 0.05).The incidence of SMT lesions in duodenum was higher in males than in females,and lower in other parts than in females.Statistical analysis of EUS results showed that there were significant differences in the occurrence rate of SMT in different parts(P< 0.05).The occurrence rate of gastric proper muscular layer was the highest,that of duodenal submucosal layer was the highest,and that of esophageal mucosal muscular layer was the highest.The composition of GIST,leiomyoma,HP and other pathological diagnosis in different parts of upper digestive tract was different,with statistical significance(P< 0.05).Conclusion: There was no significant difference in the sex ratio of upper gastrointestinal submucosal tumors;the incidence of SMT was different in different age groups,with the highest in the age group of 41-60 years old;the incidence of SMT in different sex groups was statistically significant;the incidence of SMT in duodenum was higher in males than that in females,and the incidence of SMT in other parts was lower than that in females;the incidence of SMT in different parts was statistically significant,among which gastric SMT occurred in intrinsic muscular layer The incidence of duodenal SMT was the highest in submucosa,esophageal SMT was the highest in myometrium,gastric SMT was the most common lesion in GIST,and esophageal SMT was the most common lesion in leiomyoma.The above conclusions can provide reference for clinical diagnosis and treatment.
