Benign And Malignant Pulmonary Solid Nodules By Enhanced CT Scanning And DWI Research On Diagnostic Value

Objective : To explore the diagnostic value of dynamic contrast-enhanced multi-slice spiral CT(MSCT)and DWI in benign and malignant pulmonary nodules,so as to provide more simple imaging basis for early imaging diagnosis of pulmonary nodules.Methods: Fifty-two cases of pulmonary nodules confirmed by operation and pathology or clinical follow-up in the First Affiliated Hospital of Nanchang University from January 2016 to December 2018 were collected.The imaging and clinical data of dynamic enhanced chest scan and conventional plain,multi-phase enhanced MR and diffusion-weight imaging(DWI)were retrospectively analyzed.According to the difference between benign and malignant lesions,the cases were divided into two groups: malignant group of 32 cases of lung cancer and benign group of 20 cases.The imaging data of CT and MRI were collected by image workstation and PACS system.The image features(lobulation sign,burr sign,vascular traction sign)and signal intensity of T1 WI,T2WI and DWI were collected and ADC values were measured.Number data were expressed in n% form.Generally,x_test was used.Independent two-sample t test was used to analyze the difference between the two groups in data T1 WI,T2WI,ADC value,enhancement peak value and enhancement increment(P < 0.05).Results:(1)There are many CT enhancement signs in benign group and malignant group,the probability of lobulation sign,burr sign and vascular traction sign in the former is 5%(1/20),15%(3/20),10%(2/20),and the latter is 43.75%(14/32),40.6%(13/32),68.75%(22/32),the latter is significantly higher than the former(P < 0.05);(2)Signal value: signal value of T2 WI in benign group,peak value of enhancement,strong.The mean values of chemical value-added and ADC(b=50,800s/mm2)were(240.68(+94.34),and(1.596(+0.422)x 10-3 mm/s2,respectively.The corresponding parameters of malignant lung cancer group were(305.98(+52.21),and(1.139(+0.150)*10-3 mm/s2),respectively.There were statistical differences between the above groups(P < 0.05).The signal values of T1 WI were(225.13 +40.82),(204.42 + 35.35).There was no significant difference(P = 0.093,P > 0.05).Conclusions:(1)The frequency of lobulation sign,burr sign and vascular traction sign in malignant lung cancer group was higher than that in benign lung cancer group;(2)The signal value of T2 WI,enhancement peak value and enhancement value in malignant lung cancer group were higher than that in benign lung cancer group,and the ADC value was lower than that in benign lung cancer group.There were statistical differences in these parameters between the two groups(P < 0.05);but there was no significant difference in T1 WI signal value between the two groups(P > 0.05).
