Background:Acute left heart failure is a syndrome of cardiac insufficiency caused by various heart diseases,which is the final stage of acute heart diseases caused by various diseases and seriously threatens human life and health.Heart failure has a high incidence.Heart failure is reported to be one of the most common diseases in elderly patients.Acute heart failure(AHF)is characterized by acute onset,rapid onset,severe condition and poor prognosis.With the deepening understanding of the pathogenesis of acute left heart failure,great progress has been made in drug treatment in recent years.However,the treatment of acute left heart failure attacks in modern medicine is still relatively limited,and its re-hospitalization rate,mortality rate and prognosis are still poor,which has caused a heavy economic burden to the medical and health system.OBJECTIVE :To explore the clinical effect of high dose a ldosterone antagonist spironolactone in acute left heart failure,and to compare BNP,LVEF,LVEDd,serum potassium,creatinine and other indicators were used to determine the intervention of high dose aldosterone antagonist spironolactone in the treatment of acute left heart failure to improve cardiac function and prognosis.METHODS:A total of 100 patients with acute left heart failure were selected from June 2018 to December 2018.The principle of grouping was parity of admission sequence.The patients were divided into two groups: study group(50 cases Routine treatment of high dose aldosterone antagonists plus acute left heart failure)and control group(Routine treatment of acute left heart failure in 50 cases).On admission,BNP,LVEF(left ventricular ejection fraction),LVEDd(left ventricular end-diastolic diameter),LVESd(left ventricular diastolic and systolic diameter),BNP(brain natriureticpeptide)and other indicators were obtained by color Doppler echocardiography and blood examination.After one week of admission,BNP ? LVEF were examined again.Observation of clinical symptoms of patients.Data Processing and Measurement Data Processing with SPSS16.0 Software.To compare the effect of conventional therapy and high dose aldosterone antagonist spironolactone on acute left heart failure,and further explore the feasibility of clinical application of high dose aldosterone antagonist spironolactone in acute left heart failure.RESULTS:1.Before and after treatment,the symptoms of dyspnea and other symptoms were significantly improved,blood pressure,heart rate and other indicators were significantly controlled.2.After treatment,LVEDD(left ventricular end diastolic diameter)and LVESD(left ventricular diastolic and systolic diameter)in the study group were significantly lower than those in the control group;P < 0.05(with statistical difference);after treatment,the LVEF in the study group was significantly higher than that in the control group,P < 0.05(with statistical difference).3.BNP(brain natriuretic peptide)in the study group was significantly lower than that in the control group(P < 0.05).Conclusion :1.On the basis of routine treatment,high-dose aldosterone antagonist spironolactone can significantly improve the clinical symptoms of acute left heart failure and effectively control the heart rate,blood pressure and other indicators.2.On the basis of routine treatment,high-dose aldosterone antagonist spironolactone can significantly improve left ventricular ejection fraction and reduce BNP level.It is a feasible method to alleviate acute left ventricular failure in clinic.
