Background:With the increasing morbidity and mortality of gastric cancer,gastric cancer is not only a threat to the health of Chinese residents,but also a serious global public health problem.China is a region with a high incidence of gastric cancer,with 400,000 new cases of gastric cancer each year,accounting for 42%of the total number of gastric cancer patients in the world.According to the Annual Report of Cancer Registration of China 2016,the incidence of gastric cancer ranks third in the National Cancer Registration area,reaching 33.66/100,000,accounting for 11.83%of all cancer incidence,and its mortality rate ranks third in the country,reaching 24.11/100.000.As one of the most active provinces in China’s economy,the incidence and mortality of gastric cancer in Jiangsu Province are not optimistic,ranking second in Jiangsu Province,and are higher than the overall level of the country.Sheyang County is located in the coastal area of northern Jiangsu Province.Its incidence and mortality of gastric cancer rank second in the county,second only to lung cancer.Moreover,the incidence and mortality of gastric cancer in Sheyang County exceed the average level of Jiangsu Province and the overall level of the country.The situation is not optimistic.In recent years,the state has treated the environmental pollution around Sheyang County,and implemented the policy of improving water and toilets,which has greatly improved the environmental health of Sheyang County,Jiangsu Province.Objective:The purpose of this study was to explore the main risk factors of long-term high incidence of gastric cancer in Sheyang County of Jiangsu Province through a case-control study,and to provide basic data for the prevention and treatment of gastric cancer in Sheyang County,and to analyze the correlation between nitrate load in urine and gastric cancer,so as to provide more clues for the prevention and treatment of gastric cancer.Methods:A population-based case-control study was conducted in Sheyang County,Jiangsu Province.New cases of gastric cancer in some townships from 2014 to 2018 were selected from the local cancer registration system as case group.According to the principle of frequency matching,the control group was randomly selected from all the townships where the case group was located.The control group was matched according to age and sex.A face-to-face questionnaire survey was conducted.And using Epidata 3.1 software to double entry questionnaire.At the same time,urine was collected for laboratory testing.And use SAS9.4 software for chi-square test,rank sum test,single-factor logistic regression analysis and multi-factor logistic regression analysis.Results:In this study,the case group and the control group were investigated by questionnaires,including basic information,living environment,personal habits.dietary factors,personality and emotional characteristics,disease history,family history.At the same time,urine was collected for laboratory testing.The main results are as follows:(1)Basic characteristics:This study matched according to age and sex according to frequency matching method,so the distribution of age and gender factors was the same between the two groups.The majority of the respondents were between 60-69 years old(44.44%)and 70-79 years old(42.33%).The majority of the respondents w.ere elderly,mainly male.with 76.19%male.There was no significant difference in demographic characteristics between the two groups(P>0.05).such as age.sex.nationality,educational level,marital status and occupation.The demographic characteristics of the case group and the control group were well balanced.(2)Univariate logistic regression analysis showed that family annual income of more than 20,000 yuan is negatively correlated with gastric cancer compared with family annual income of less than 20,000 yuan.Increased daily drinking water,moderate food temperature,longer eating time.more than once a week consumption of chili,more than once a week consumption of raw garlic,more than once a week consumption of soybean products are also negatively correlated with gastric cancer.Overweight,salty diet,drinking raw water,eating pickles more than once a week,smoking,exposure to second-hand smoke.drinking,usually unhappy mood,anger,experience unfortunate events,family history of gastric cancer,gastric diseases.Helicobacter pylori infection and other factors are positively correlated with gastric cancer.This study did not find any correlation between living environment factors,eating pork,beef and mutton,fresh vegetables and fruits.drinking tea and urine nitrate level and gastric cancer.(3)Multivariate logistic regression analysis showed that overweight(BMI≥24),salty diet,exposure to secondhand smoke,eating pickles more frequently than once a week,experiencing unfortunate events,suffering from stomach diseases were positively correlated with gastric cancer,while BMI<18.5,eating chili more frequently than once a week,eating raw garlic 1-3 times a week,and eating soybean products 1-3 times a week were negatively correlated with gastric cancer.Conclusion:Gastric cancer is the result of a combination of different factors.According to the case-control study,Overweight,salty diet,exposure to second-hand smoke,frequent consumption of pickles,unfortunate events and gastric diseases may increase the risk of gastric cancer in Sheyang County,Jiangsu Province.Therefore,we should strengthen physical exercise to lose weight,:maintain good living habits and eating habits.Eating less pickles.Stop smoking and limit alcohol,ensure physical and mental pleasure,thereby reducing the incidence of gastric cancer.
