OBJECTIVE: With the rapid development of modern medicine,Percutaneous coronary intervention has become one of the main treatments for acute myocardial infarction.However,there are many complications after PCI,such as in-stent restenosis and acute or chronic inflammatory reactions.Panax notoginseng saponins extracts from traditional Chinese medicine Sanqi,which has the functions of promoting blood circulation,removing phlegm and relieving pain.The preparations are widely used in clinical treatment,but the evaluation of anti-thrombotic and inflammation of PNS on AMI patients after PCI was rarely reported.Therefore,this study is designed to explore the effect of PNS on the antithrombotic and inflammatory response on AMI patients after PCI.METHODS: Seventy patients diagnosed with acute myocardial infarction who were admitted to the Department of Cardiology,Central Theater General Hospital from July 1,2017 to March 31,2018,were divided into two groups according to random allocation.There were 35 cases in the control group and 35 cases in the experimental group.There were 2 cases due to interference from other factors and data loss,at last,68 cases were actually included,34 cases in the control group,and 34 cases in the experimental group.Both g roups of patients met the diagnostic criteria of AMI and blood stasis syndrome.All patients underwent emergency PCI after admission,including coronary angiography,percutaneous transluminal coronary angioplasty and coronary stenting.After the operation,the control group was given treatment of anti-platelet aggregation,stable plaque,dilated coronary artery,inhibition of ventricular remodeling,etc.On the basis of the control group,the experimental group given Xueshuantong injection 0.5g(mainly PNS)dissolved in 0.9% sodium chloride injection 250 ml,40 drops / min,once a day,the total treatment of Xueshuantong injection was 10 days.Platelet activation markers(CD62P,PAC-1),platelet parameters(PLT,MPV,P-LCR,PDW),coagulation parameters(D-dimer,FIB)and inflammatory markers(IL-6,hs-CRP)were measured on the first and 11 th day after PCI.Observed whether there were any adverse events such as hemorrhage during the treatment,and evaluated the efficacy of TCM symptoms on the 11 th day after PCI.Results: 1.The level of CD62 P and PAC-1 on the 11 th day of the two groups were lower than the first day after PCI.On the 11 th day after PCI,the level of CD62 P and PAC-1 in the experimental group were lower than the control group,There were differences between two groups(P< 0.05).2.The level of PLT on the 11 th day of the two groups were higher than the first day after PCI.The level of MPV,P-LCR,and PDW were decreased.And the level of MPV and P-LCR in the experimental group were lower than those in the control group on the 11 th day after PCI.There were significant differences between two groups(P<0.05).3.The level of D-dimer and FIB on the 11 th day of the two groups were lower than the first day after PCI.On the 11 th day after PCI,the level of D-dimer and FIB in the experimental group were lower than the control group,There were differences between two groups(P<0.05).4.The level of hs-CRP and IL-6 on the 11 th day of the two groups were lower than the first day after PCI.On the 11 th day after PCI,the level of hs-CRP and IL-6 in the experimental group were lower than the control group,There were significant differences between two groups(P<0.01).5.The bleeding events in the two groups were comparable during the treatment,there were no differences between two groups(P>0.05).Conclusion: For patients with myocardial infarction and blood stasis syndrome,PNS can exert antithrombotic effect by inhibiting platelet activation,reducing platelet volume,reduce platelet hyperresponsiveness,improving hypercoagulability of the body,and inhibiting the postoperative inflammatory response of PCI in patients with AMI.And it did not increase the incidence of clinical bleeding events.Therefore,it can be used as a routine medication for patients with AMI and blood stasis syndrome after PCI.
