Regulation Effect Of Huai Qi Huang Granules On Tfh Cells And Tfr Cells Of Children With Epstein-Barr Virus-Associated Infectious Mononucleosis

ObjectiveTo investigate the effects of Huaiqi Huang Granule on CD4 + T and CD8+T cells in children with EBV-related infectious mononucleosis(IM)and the possible regulatory mechanism and significance of Huaiqi Huang Granule on peripheral blood follicular regulatory T cells(Tfr cells)and follicular helper T cells(Tfh cells),so as to provide a basis for clinical immunoregulation in children with EBV-IM Definite basis.MethodsThe subjects were children with suspected EBV-IM who were admitted to our hospital from February 2017 to February 2018.After admission,routine physical examination and blood routine,liver and kidney function,EBV detection,lymphocyte type,liver and spleen color Doppler ultrasonography were performed.A total of 152 cases were finally diagnosed.Combined with TCM syndrome differentiation,children with deficiency of both Qi andYin were included.In this study,all children who participated in the study participated in the study when their parents or guardians signed the informed consent.The basic data of each child(age,sex,etc.)were recorded in detail.The children were divided into two groups by random sampling,38 cases in each group.Group A: 38 cases were given ganciclovir injection 5 mg/kg,intravenous drip,Q12 h,each drip time was more than 1 hour;Group B: 38 cases were given Huaiqi Huang Granule orally on the basis of Group A;the course of treatment was 7-14 days;Group C: 25 healthy children in the healthy control group,who underwent physical examination at the same time in our hospital,had no history of immunodeficiency.Peripheral blood samples were collected before and after treatment,and lymphocyte classification and flow cytometry were performed to analyze the ratio of CD4 +,CD8 + in peripheral blood and the expression of Tfh cells and Tfr cells.The changes of immune function in children before and after treatment were monitored and analyzed.Results1.A total of 69 cases of infectious mononucleosis with deficiency of both Qi and Yin were collected in this study.Two of them terminated the test program because of the progress of the disease.One child refused to cooperate with the treatment during the course of treatment.After parents asked to terminate the test program,a total of 66 cases were included,including 40 males and 26 females,aged 2-13 years,with an average age of(4.7 +2.3).Group A consisted of 34 patients,including 22 males and 12 females,aged from 3 to 10 years,with an average age of(4.7 + 2.1)years.Group B had 32 cases,including 18 males and 14 females.The age ranged from 3 to 12 years,with an average of(4.5 + 2.4)years.There were 25 cases in group C,15 males and 10 females,aged from 3 to 12 years,with an average age of(4.5 + 2.4)years.There was no significant difference in gender and age among the three groups(P > 0.05),which was comparable.2.Before treatment,CD4 + T cells in peripheral blood of children in group A and B were significantly decreased,CD8 + T cells were significantly increased,and the ratio of CD4 +/CD8 + was significantly inverted.There was no significant difference between the two groups(P > 0.05),while the ratio of CD4 + T cells,CD8 + T cells and CD4 +/CD8+ in group C was normal.There was significant difference between group A and group B and group C(P < 0.05).After10 days of treatment,the number of CD4 + T cells,CD8 +T cells and the ratio of T cells(CD4 +/CD8 +)between group A and group C were significantly different(P < 0.01).There was no significant difference in the number of CD4 + T cells,CD8 + T cells and the ratio of T cells(CD4 +/CD8 +)between group B and group C(P > 0.05).3.Before treatment,the proportion of Tfh cells in peripheral blood of EBV-lM children in group A and B was significantly higher than that in group C,the proportion of Tfr cells was significantly lower than that in group C,and the ratio of Tfh/Tfr was significantly higher than that in group C(P < 0.05).4.Ten days after treatment,there were significant differences in the proportion of Tfr cells,Tfh cells and Tfh/Tfr ratio before and after treatment in group B(P <0.05),but there were no significant differences in the proportion of Tfr cells,Tfh cells and Tfh/Tfr ratio before and after treatment in group A(P > 0.05).Longitudinal comparison showed that after treatment,the proportion of Tfh cells and Tfh/Tfr ratio in group B were significantly lower than those in group A,and the proportion of Tfr cells was significantly higher than that in group A.There was significant difference among the three groups(P < 0.05).Conclusion:Huaiqi Huang Granule can maintain the cellular immune function of the body by maintaining the number of CD4+T cells in EBV-IM children.On the other hand,it can also increase the number of Foxp3+Treg cells,promote the differentiation of Tfr and inhibit the auxiliary role of Tfh cells,thus playing an immunoregulatory role in order to reduce the immune damage of the body.
