Objective:To explore the difference of clinical manifestation,specificity,treatment and prevention between(HLAP)and(ABP)in hyperlipidemic acute pancreatitis,in order to provide reference basis for early diagnosis,correct treatment and prognosis of HLAP.Method:From January 2013 to January 2018,37(HLAP)patients with hyperlipidemic acute pancreatitis who were hospitalized in our department were selected as the observation group,and 71(ABP)patients with acute biliary pancreatitis who were hospitalized in our department at the same time were randomly selected as the control group.The general clinical data,laboratory and instrument results,concomitant diseases,complications,treatment and prognosis were compared between the two groups.Result:(1)the average age of HLAP group was significantly lower than that of ABP group(P < 0.05).There was no significant difference in sex distribution between the two groups(P > 0.05).(2).The Proportion of obesity,diabetes mellitus and fatty liver in the HLAP group was significantly higher than that in the control group(P < 0.05).There was no significant difference in the prevalence of hyPertension between the two groups(P > 0.05).(3).There wasno significant difference in the clinical manifestations and signs between the two grouPs.(4)the serum triglyceride(TG)in the HLAP group was not specific.The level of total cholesterol(TC),low density liporotein(LDL-C)in(TG),was significantly higher than that in ABP grouP(P < 0.05).There was no significant difference in high density lipoprotein(HDL-C)level between the two groups(P > 0 05).Among the 37 patients with HLAP,22(59.4%)had higher serum CRP and PCT levels than those in the control group(> 11.3mmol / L ·(5)HLAP)(P < 0.05),and 19 Patients(47.5%)had significantly higher levels of serum CRP and PCT than those in the control group.It is suggested that the level of inflammation in patients with HLAP is higher.(6)the index of liver enzyme(AST,ALT,GGT)in patients with ABP is higher.And bilirubin(total bilirubin,direct bilirubin)were significantly higher(P < 0.05).There was no significant difference in serum amylase level between the two groups,but the serum amylase level in ABP group was significantly higher than that in HLAP group(P< 0 05),but there was no significant difference between the two groups(P > 005).The level of blood glucose in HLAP group was higher than that in HLAP group(P < 0.05).(6).The rate of pleural effusion and pulmonary infection in HLAP group was higher than that in control group(P < 0.05).There was no significant difference in the incidence of other complications between the two groups(P > 0 05).(7).There was no significant difference in and APACHE-II between the two groups.No significant difference was observed in the score(P > 0 05),but the BalthazarCT score in HLAP group was higher than that in HLAP group(P > 0 05),and the Percentage of SAP in HLAP group was higher than that in HLAP group.The average length of stay in hosPital was slightly longer than that in ABP group(all P < 0 05).(8),but the rate of operation in ABP group was significantly higher than that in ABP group(all P < 0.05 and P< 0.05,P < 0 05).During one-year follow-up,the recurrence rate and average recurrence times in HLAP group were higher than those in ABP group(P < 0.05).Conclusion:In recent years,HLAP is more and more common in clinical practice.Both HLAP and ABP Patients are mainly male.However,the incidence of HLAP has an obvious tendency of younger age,and the disease is often more serious,and pulmonary complications are more common.Obesity and other basic diseases,so we should pay attention to the control of blood liPids,blood sugar levels and weight,in order to prevent the induction of HLAP or recurrence.The detection of serum amylase is of limited value in the diagnosis of HLAP.Patients with normal or slightly elevated serum amylase may not completely rule out the Possibility of HLAP,while dyslipidemia is a characteristic indicator of HLAP.HL AP recurrence rate is higher,HLAP patients should also pay attention to the control of blood lipid after discharge to minimize the risk of recurrence.
