A Clinical Annalysis Of Pressure Ulcer

Objective To investigate the causes of pressure ulcer,analyze the surgical treatment and recovery of pressure ulcer in our hospital in recent years,and evaluate the efficacy of treatment of pressure ulcer.Methods We collected the details of 76 patients(122 pressure ulcers)with pressure ulcers in the Department of Burn and Plastic Surgery of the First Affiliated of Guangxi Medical University from October 2012 to December2018.And analyzed the causes and clinical characteristics of pressure ulcer.Surgical treatments were carried out according to the clinical characteristics of ulcer.The healing of pressure ulcer was evaluated by PUSH scale.learn the curative effect of surgical treatment of pressure ulcer.Results(1)The main causes of pressure ulcer were traumatic paraplegia.(2)Distribution of age and sex: Men are more common in the group of pressure ulcer.A total of 61 cases,accounting for 80.2% of the total,15 cases of women,accounting for 19.7% of the total.Among 76 patients,the youngest was 10 years old and the oldest was 84 years old.The average age was 53.1years old.Among them,51 cases(67.1%)were 40-65 years old,and 25 cases(19.7%)were over 65 years old.(3)The main sites of pressure ulcer were buttocks and sacrococcyx,accounting for 80.8% of the total cases.(4)Nutritionstatus of patients with pressure ulcer In the group,23 patients(30.2%)had serum albumin < 30g/L,44 patients(57.8%)had serum albumin < 35g/L and 32patients(42.1%)had serum albumin < 35g/L.(5)PUSH score of operation and wound healing:In this group,114 operations were performed,of which 43 cases(56.5%)had primary wound healing after one operation.One case was discharged from hospital automatically because of sepsis after one operation.13cases(17.1%)healed after two operations,and 4 cases(5%)healed after three operations.After three operations,the wound improved and was discharged from hospital.One patient died of sepsis after three operations.The wounds improved in 3 cases after 4 operations and healed in 1 case after 5 operations.The total healing rate was about 75%.At discharge,22 ulcers(18.4%)had PUSH score of0;51 ulcers(42.8%)had PUSH score of 1-4;13 ulcers(10.6%)had PUSH score of 5-6;11 ulcers(9%)had PUSH score of 7-10;23 ulcers(19.3%)had PUSH score of more than 10.Conclusion(1)Traumatic paraplegia is one of the main causes of pressure ulcer;(2)45-65 years old is the most common age group of pressure ulcer patients,and male is the most common;(3)Surgery can significantly improve the wound healing rate of pressure ulcer stage IV,which is the main method to treat pressure ulcer stage IV;(4)poor nutritional conditions is the important factor affecting the healing of pressure ulcer;(5)PUSH scale can be used to evaluate the efficacy of wound healing after pressure ulcer surgery.
