Study On The Correlation Between Yin Syndrome Ulcer Care And Wound Healing And The Mechanism Of Traditional Chinese Medicine Intervention

Chronic refractory wounds usually refer to the wounds that can not be repaired in an orderly and timely manner to achieve anatomical and functional integrity under the action of various internal and external factors,and enter a pathological inflammatory reaction state.Chronic refractory wounds have become a hot and difficult point in the field of wound repair because of their great individual differences,easily repeated conditions,treatment cycle and difficulty.In traditional Chinese medicine surgery,this kind of refractory wound belongs to the category of "Yin syndrome ulcer".Traditional Chinese medicine has a long history in wound repair and has a remarkable curative effect.This study summarizes the traditional knowledge of wound healing rules and treatment methods of traditional Chinese and Western medicine,and observes the curative effect through literature research and self-control experiment design.It focuses on the phenomenon of blood stasis around wound in chronic wound repair,and puts forward problems and related research directions.Firstly,we define this stagnation phenomenon as a protective field,and consider it as a unique form of existence in the protective field of Yin ulcer.Secondly,we inherit and develop the traditional theory of protecting the ulcer and ulcer in traditional Chinese medicine,and put forward the "theory of protecting the ulcer and ulcer of yin" which is applicable to the ulcer of Yin sy ndrome,and study its objective phenomena and laws.In order to observe the clinical effect and mechanism of pressure ulcer wound healing,two to three stages of pressure ulcer protector were applied with "pressure ulcer medicine wine" featuring traditional Chinese medicine.This paper puts forward and explains the significance of the theory of Yin-syndrome ulcer protection in the treatment of Yin-syndrome ulcer represented by stress injury,and further explains the theory of Yin-syndrome ulcer protection and its clinical significance in various types of Yin-syndrome ulcer,so as to contribute to improving the clinical efficacy and improving the repair system of chronic wounds.Objective:Based on the theory of Yin-syndrome ulcer protection field,the clinical effect of pressure ulcer medicine liquor on the healing of pressure ulcer wound in stage 2-3 was observed,and its mechanism was preliminarily studied.Methods:Thirty patients with stage 2-3 pressure ulcer were enrolled from January 2018 to January 2019.Two pressure ulcers of patients were evaluated by self-controlled trial design.They were divided into experimental group and control group.By comparing the healing of different interventions,we explored the clinical effect of pressure sore medicinal liquor on the healing of pressure sore wounds in stage 2-3 with the blood stasis zone around the wound.We also compared the changes of laser Doppler blood flow before and after the intervention in the experimental group,and preliminarily explored the mechanism of pressure sore medicinal liquor.Results:In this study,30 patients with pressure ulcers were included,each of them had 2 pressure ulcers in stage2-3,16 in males(53.33%)and 14 in females(46.67%).The age of patients ranged from 56 to 82 years old,with an average age of 72.28±7.16 years.In the experimental group,30 cases of pressure sores were treated by debridement of the wound surface,external application of Jiedu Shengji Ointment combined with skin-benefiting active growth factor,In the control group,30 cases of pressure ulcers were treated with traditional Chinese medicine,Jiedu Shengji Ointment and Yimei Active Growth Factor.Result:The average healing area of the treatment group was(24.7±17.55 cm2)and that of the control group was(14.7±9.44 cm2).The average healing area of the experimental group was larger than that of the control group,and the difference was statistically significant(t =2.748,P=0.009,P<0.05).The mean shrinkage area of the stagnation zone in the experimental group was(55.13±43.32 cm2)and that in the control group was(26.4±18.95 cm2),with significant difference(t=3.329,P=0.001,P<0.05).Before treatment,the Doppler blood flow was(62.70±13.03)and during treatment,the mean was(85.50±16.84).The difference was statistically significant(t=3.386,P=0.003,P<0.05).Compared with the treatment,the laser Doppler blood flow value after treatment was(59.74±13.22);the difference was statistically significant(t=3.804.P=0.001,P<0.05).Conclusion:Chronic refractory wounds have the phenomenon of blood stasis around the wound.We define this phenomenon as the protective field of Yin ulcer.With2-3 stages of stress injury as the research object,pressure ulcer medicinal liquor was applied to the pressure ulcer protective field.It was proved that pressure ulcer medicinal liquor could significantly improve the state of peri-traumatic Stasis Zone of pressure ulcer and promote wound healing when improving the state of peri-traumatic stasis zone.Laser Doppler blood flow value in pressure ulcer medicine wine rubbing pressure ulcer protective field,the trend is low-high-low,and the numerical changes have statistical significance.Pressure sore medicinal liquor can increase blood perfusion around the wound and wound by intervening in the peri-wound nursing ground of pressure sore.Therefore,it can be proved that there is a protective field in Yin ulcer,and it can promote the repair of Yin ulcer by intervening in Yin ulcer protective field.
