Objective: To select the index which can evaluate the risk degree of primary medical and health personnel by Delphi method,and to construct the medical risk assessment system of primary medical and health personnel.Thirty primary medical institutions in Guiyang City,Guizhou Province,were selected for empirical research to test the practicability and applicability of the index system.Methods: The main results are as follows:(1)The preliminary index system is screened by Delphi expert consultation method(Delphi Method),and the reliability of this study is analyzed by calculating the positive coefficient,authority coefficient and coordination coefficient of experts.The threshold method is used to screen the index,and the weight of the index is established based on the fuzzy comprehensive evaluation method.(2)A questionnaire is set up according to the index system to carry out an empirical investigation on 30 primary medical institutions in Guiyang.A total of 300 patients and 325 primary health care personnel covered by the institutions involved were investigated,and the survey number was investigated by Epidata3.0.According to the collation,the reliability and validity of SPSS19.0 questionnaire are used to test the index system.(3)According to the empirical results,the risk safety index of primary medical and health personnel is established,and the comprehensive index score of medical risk and safety of primary medical and health personnel is calculated.To evaluate the personal risk level of primary medical and health personnel.(4)Based on the index formula,the risk assessment system program is established by python language.Results:The main results are as follows:(1)After two rounds of Delphi expert consultation,the medical risk evaluation index system of primary medical and health care personnel is established,and the reliability of the index is analyzed according to the positive degree,authority degree and expert coordination degree of expert consultation.In the two rounds of expert surveys,the positive coefficient of experts is 100%,the authority coefficient is 0.83,and the coordination coefficients of the two rounds of experts are 0.342 and 0.476,respectively,which meet the requirements of Delphi expert consultation method.The results of the consultation are reliable.(2)The ? coefficients of the three questionnaires are 0.859,0.850 and 0.829,respectively,indicating that the reliability of the questionnaire is good.Using KMO test,Bartlett's spherical test and principal component analysis were used to test the structural validity of the questionnaire for patients and medical and health personnel.The KMO of patient questionnaire is 0.760,?2=984.307,df=78,P<0.001,and the KMO of questionnaire for primary health care workers is 0.850,?2=3285.331,df=300,P<0.001.The reliability and validity tests meet the statistical requirements and verify the objectivity and effectiveness of the index system.(3)According to the empirical results,the comprehensive index of medical risk and safety of primary medical and health personnel in Guiyang City is put forward.The risk assessment system of primary medical and health personnel is established by python language,and the comprehensive index score of medical risk and safety of primary medical and health personnel in Guiyang City is calculated according to the index model.The results showed that 76% of the primary medical and health workers had a medical risk level of less than 20 points.Conclusion: The main results are as follows:(1)The index system is constructed by Delphi expert consultation method and fuzzy comprehensive evaluation method is adopted to improve the objectivity of the index.The empirical results show that the index system has high reliability and validity.It has high practical value in practical application.(2)The risk assessment system of primary-level medical personnel established by python language can help medical institutions to evaluate and analyze the risk level of medical staff according to their own situation.In order to improve the medical institutions risk management level and evaluation efficiency.
