Background:Non-primary head and neck malignancy refers to a malignant tumor whose primary lesion is located outside the head and neck and then metastasized to the head and neck.The incidence of this disease is low,clinically rare,lack of relevant diagnostic experience,clinically easily missed diagnosis,misdiagnosis,the current diagnosis of this disease mainly depends on pathological results and imaging examination,the tumor has been found when distant metastasis,That is,in the advanced stage of the tumor,the treatment plan is mainly surgery,and adjuvant chemotherapy and radiotherapy.For patients who do not have surgical indications,radiotherapy and chemotherapy are preferred,and the treatment of primary disease can not be ignored.Therefore,it is especially important to reduce misdiagnosis,missed diagnosis,early detection and treatment.Objective:Through relevant case analysis,master the clinical characteristics,diagnostic methods and treatment plans of these diseases,and continuously summarize the experience in the clinical,deepen theunderstanding of clinicians such diseases,improve the diagnostic level,reduce the occurrence of misdiagnosis,missed diagnosis,and timely precision Treatment to improve patient survival and quality of life.Methods:Patients from the Department of Otolaryngology Head and Neck Surgery of the Second Hospital of Jilin University from October 2013 to October 2018 were collected.The pathological results were supported by cases of distant metastatic malignant tumors of the head and neck,and the cases were complete.Five representative cases were included,and the basic data,clinical manifestations,laboratory tests,imaging examinations,pathological results,and treatment follow-up were summarized.Results:Most of the patients are elderly,metastatic tumor is located in the head and neck,and the pathological results support metastatic malignant tumor.Considering the comprehensive situation of the disease,individualized treatment is implemented according to the condition and the individual wishes of the patients.For metastatic tumors with limited lesions,surgical resection is preferred.Surgical resection can improve the patient's survival rate,but recurrence is still possible.For patients who cannot undergo surgery or refuse to receive treatment,the prognosis is poor,with the minimum death of one month.Conclusions:1.For patients with head and neck masses,we should carefully review the medical history,view imaging data,comprehensively assess thepatient's condition,and be alert to distant metastatic tumors.2.The pathological results can indicate the location of the primary tumor to a certain extent,providing strong evidence for the final comprehensive diagnosis.3.Patients with head and neck tumors should be routinely examined by chest and abdomen CT,comprehensive assessment of the condition,and metastatic tumors should be performed.PET-CT should be performed to assess the metastasis and reduce misdiagnosis.4.The limitation of metastatic tumor lesions,the first choice for surgical treatment,postoperative adjuvant chemotherapy and primary tumor treatment,and chemotherapy for patients who cannot and cannot undergo surgery.The assessment of the primary tumor is extremely important.It determines the treatment plan for this tumor.Therefore,it is especially important for the treatment to comprehensively assess the condition and avoid missed diagnosis and misdiagnosis.
