Purpose:To observe whether acupuncture facilitation technique of pricking acupoints on the feet can improve the lower limbs motor function of poststroke patients of Brunnstrom ?-?period(especially ? period).Methods:51 patients accordance with inclusion criteria were divided into 6 groups according to random number table and Brunnstrom grading.There are two discussion plans:?-? stage is composed of Bruunstrom ?-? stage treatment group(25)and control group(26).? stage is composed of Bruunstrom ? stage(course in 1 month)treatment group(19)and control group(14).All patients in both trials received conventional drug therapy and rehabilitation.The control group adds the traditional acupuncture therapy:choose acupoints of affected side upper limb(LI15,LI11 LI10,SJ5,LI4),affected side lower limbs(GB30,GB34,ST36,ST41,BL60).The treatment group received the same upper limb acupuncture on the affected side.But the feet acupoints were performed only by pricking on the feet.All patients were treated once a day for 6 days.The outcomes were assessed before treatment(DO),3 days after treatment(D3)and 6 days after treatment(D6).Main evaluation indexes:1.Brunnstrom assessment(Ueda Satoshi);2.Fugl-meyer lower limbs motor function score(FMA-LE);3.Hoffer's ambulation classification;4.Holden walking function classification.Secondary evaluation indexes:1.Active Range of Motion-Lower Limb(AROM-LL),2.Assessment of lower limb muscle tone(MAS),3.Assessment of lower limb unarmed muscle strength(MMT).Outcomes:In the experiment,multiple positive results of self-controlled before-after study were found in both the control group and the treatment group.In comparison between the two groups,there were a number of positive results,as follows:1.In the I-IV stage:Brunnstrom assessment(P=0.013);Total score of FMA-LE after D3 and D6(P<0.01);The section B,C,D,E,G of FMA-lE-ss after D3 and D6(P<0.01/P<0.05);Hip flexion ROM after D3 and D6(P<0.05),Hip extension ROM after D3 and D6(P<0.01),Knee flexion ROM after D3 and D6(P<0.05),Knee extension ROM after D3 and D6(P<0.05),Ankle flexion ROM after D3 and D6(P<0.05),Ankle extension ROM after D3 and D6(P<0.01);Hip flexion MMT(P=0.038),Knee flexion MMT(P=0.048).2.In the III stage(course in 1 month),the positive results between the two groups were Brunnstrom assessment(P=0.000);Total scores of FMA-LE after D3 and D6(P<0.01),The section B,C,D,G of FMA-lE-ss after D3 and D6(P<0.01/P<0.05);Hoffer walking ability rating(P=0.020);Holden walking function classification(P=0.011);Hip flexion ROM after D3 and D6(P<0.01),Hip extension ROM after D3 and D6(P<0.01),Knee flexion ROM after D3 and D6(P<0.05),Ankle flexion ROM after D6(P<0.05),ankle extension ROM after D6(P<0.01);Hip flexion MAS(P=0.038);Hip flexion MMT(P=0.007),Knee flexion MMT(P=0.020),Knee extension MMT(P=0.028).Through the above positive results,it suggested that the self-controlled before-after intervention in the control group and treatment group indeed has promoting effect in motor function of lower limbs of the Brunnstrom I-IV period patients after stroke.From the ? stage(course in 1 month),between the two groups of results suggested that pricking acupoints of feet can effectively improve the recovery of motor function of lower limbs after stroke.Within a week,it is helpful to rapidly improve the recovery of lower limb motor dysfunction after stroke and promote the evolution process of rehabilitation.Conclusion:1.The facilitation technique of pricking acupoints on the feet can apply to treat the lower limbs motor dysfunction after stroke.It has definite improvements on the patients of Brunnstrom ?-? period(especially in stage III).2.It also has a good promoting effect on the balance stability,the ability to control the movement coordination by the improvement of the extension ability of the lower limbs.So as to provide a new treatment idea and method for rapidly improving the recovery of lower limbs motor function after stroke in clinical study.
