Therapeutic Effect And Mechanism Of Electroacupuncture At Deep Acupoints On Spleen And Kidney Yang Deficiency Ulcerative Colitis

Objective:To investigate the clinical efficacy of electroacupuncture at deep acupuncture at Baji point in the treatment of ulcerative colitis of spleen and kidney yang deficiency,and to explore its mechanismMethods:The patients with anorectal diseases of Nanjing Chinese Medicine Hospital affiliated to Nanjing University of Traditional Chinese Medicine from June 20 to December 30,2018 were diagnosed as ulcerative colitis,and 30 patients with spleen and kidney yang deficiency syndrome and met the inclusion and exclusion criteria.’They were randomly divided into control group(n=15)and treatment group(n=15).The control group was treated with mesalazine granules.The treatment group was treated with western medicine on the basis of western medicine.Acupoints(middle,lower jaw),both groups of treatment are 2 weeks.The clinical efficacy,TCM total score,IBDQ quality of life scale,modified Mayo score,C-reactive protein,erythrocyte sedimentation rate,fecal calprotectin and heart rate variability were observed and analyzed.Results:1)Clinical effective rate:The clinical effective rate of the treatment group was better than that of the control group after treatment,the difference was statistically significant(P<0.05);2)The total score of TCM symptom after treatment and the total score of IBDQ quality of life scale after treatment Before the two groups were treated,the two groups had a significant decrease after treatment(P<0.05).The total score of TCM symptoms after treatment,the total score of IBDQ quality of life scale and the two total points after treatment in the control group.Compared with the difference,the difference was statistically significant(P<0.05).3)The modified Mayo score,CRP,ESR and FC were lower in the two groups than before the two groups,and the two groups decreased significantly after treatment compared with before treatment(P<0.05);the modified Mayo score,CRP,ESR and FC after treatment in the treatment group were statistically significant(P<0.05);4)Autonomic function:heart rate variability through ECG The spectral analysis of(HRV)showed that the HRV(L F/I I F)of the two groups decreased after treatment,but the treatment group was superior to the control group(P<0.05).The acute electroacupuncture at the deep acupoints increased the vagus nerve activity(P<0.05)and reduced sympathetic nerve Activity(P<0.05);chronic electroacupuncture at deep acupoints showed similar enhancement of vagal activity(P<0.05)and inhibition of sympathetic activity(P<0.05);5)No adverse reactions were found during the treatmentConclusion:1)Electroacupuncture at deep acupoints can improve the clinical symptoms of spleen-kidney yang deficiency ulcerative colitis;2)Electroacupuncture at deep stagnation of Ba:acupuncture may acti.vate cholinergic anti-inflammatory pathway through excitatory vagus nerve Therapeutic effect,which provides a basis for acupuncture treatment of ulcerative colitis.
