Objective Sepsis is a life-threatening organ dysfunction resulted from disordered response of the body to infection,which can induce infectious shock and multiple organ failure(MOF),finally leading to patient death.The myocardial injury of sepsis is one of the risk factors leading to increased mortality in patients;besides,the mortality of sepsis patients combined with myocardial injury is increased by 70-80% compared with those with no myocardial injury.Sepsis combined with myocardial injury can manifest as cardiac dysfunction in various forms;yet myocardial contraction dysfunction remains the leading type at present.Sepsis combined with myocardial injury is mainly diagnosed through myocardial injury markers and echocardiography at present,while heart-type fatty acid binding protein(h-FABP)is a novel myocardial marker that has been extensively applied in acute coronary syndrome,but it has not been widely recognized in sepsis combined with myocardial injury.This paper aimed to to observe the diagnostic value of h-FABP in the early diagnosis of sepsis combined with myocardial injury,so as to assist in clinical diagnosis and treatment.Methods From January 2017 to June 2018,60 sepsis patients were admitted and treated in Intensive Care Unit of Hospital;subsequently,the patients were divided into myocardial injury and non-myocardial injury groups according to the left ventricular ejection fraction(LVEF)? 50%(which revealed combined myocardial injury).Common medical manipulations,such as gastric tube,urinary catheter,central venous catheter and arterial blood pressure monitoring tube,had been implemented according to the patient conditions.With regard to clinical treatment,the primary diseases in patients were treated,and anti-infection as well as sufficient fluid infusion was alsogiven;besides,the sepsis-related complications were prevented and treated,and other conditions were given symptomatic treatment.Moreover,patient age,sex,etiology,infection site and the APPCHE II score were recorded.At the same time,basic vital signs,including body temperature,respiration,heart rate or pulse rate,and blood pressure,should also be monitored conventionally among the sepsis patients.The SPSS17.0 statistical software was adopted for all statistical analyses of data.Measurement data were expressed as mean ± standard deviation(x ± s),and paired t-test was employed for inter-group comparison,while enumeration data were analyzed by chi-square test.A difference of P<0.05 was deemed as statistically significant.The receiver operating characteristic(ROC)curve of each experimental index was plotted,so as to evaluate the diagnostic value of the index in myocardial injury.Conclusions Sepsis patients are associated with the risk of early myocardial injury.At the time of sepsis injury,h-FABP is the same with the traditional myocardial marker,which is of certain significance in sepsis combined with myocardial injury.It is discovered based on the comparisons of sensitivity and specificity that,h-FABP can serve as the serological index for the early monitoring of myocardial injury,which can better predict the risk of incidence of sepsis-combined with myocardial injury.More importantly,it is superior to the traditional CK-MB and cTnI detection,which is of better diagnostic value for myocardial injury in sepsis.
