Objective: to evaluate the type meta analysis method heart-type fatty acid binding protein(H-FABP)compared with troponin I(cTnI)and creatine kinase isoenzyme(CK-MB)in coronary artery bypass surgery perioperative period early prediction application value of myocardial injury. Methods: computer retrieval of Cochrane Library,PubMed,EMbase,CNKI,VIP and WanFang Data were retrieved from the Library to December2017.To retrieve the relevant journals by hand and trace the references in the literature.Into H-FABP and CK,MB and cTnI monitoring comparing cabg perioperative myocardial injury of cohort and case-control study,by two part evaluation shall be carried out in accordance with the include exclusion standard independent information extraction, quality evaluation and cross check, using RevMan5.3 Meta analysis software.Results:h-fabp was compared with cTnI and ck-mb in 1h,2h,3h,4h peak level, andP <0.00001.Conclusion: Meta-analysis of H-FABP and cTnI in on-pump CABG indicated that H-FABP could reach the peak level early after surgery compared with cTnI [MD=9.28,95%ci(8.08,10.48),P<0.00001].Compared with CK-MB,H-HABP can reach the peak level 2h after surgery MD=-4.41,95%CI(-6.88,-1.93),P<0.0005].In off-pump CABG surgery,H-FABP reached the peak level 3h after surgery compared with cTnI [MD=4.31,95%ci(3.69,4.92),P<0.00001].H-FABP can reach the peak level 2h after surgery compared with CK-BM,and gradually fall back [MD=0.87,95%ci(0.09,1.66),P<0.03].Conclusion: H-FABP can diagnose the myocardial injury as early as possible during the perioperative period of On-pump CABG or Off-CABG provide reliable diagnostic basis for clinical practice, and guide the early prevention and treatment of patients with myocardial injury.
