Discussion On The Optimal Scheme Of Acupuncture Treatment Of Limb Motor Dysfunction During Stroke Recovery Based On Orthogonal Design Test

Abstract/Summary:
Objective: Based on orthogonal design experiments,four factors(acupuncture retention time,needle acupuncture frequency,needle thickness,and acupuncture depth)in acupuncture treatment of limb motor dysfunction were studied.Each factor was taken at 3 levels(the needle retention time was selected).20 min,30min,40 min,the frequency of acupuncture is selected once every other day,once a day,twice a day;the thickness of the needle is 0.25 mm,0.3mm,0.35mm3;the depth of acupuncture is 3,shallow,medium and deep.The clinical trials were arranged by orthogonal design method,and the optimal plan for acupuncture treatment of limb motor dysfunction in patients with stroke recovery was screened out.Methods: According to the factors and levels of the experiment,the L9(34)orthogonal table was used.90 patients with satisfactory stroke recovery were randomly divided into 9 groups,including Acupuncture retention time(A),Acupuncture frequency(B),needle thickness(C),and Acupuncture depth(D).The corresponding 3 levels were treated.The total time of treatment in each treatment group was 2 weeks.The TCM symptom classification and quantification table(TCM score),neurological deficit score,Fugl-Meyer(FMA)score before and after treatment were observed.The Barthel index(Barthel Index,BI)changes the indicators,and the data are statistically processed to determine the pros and cons of the factors examined in this trial.Results: The influence of needle retention time on TCM syndrome scores had a great influence on patients’ TCM syndromes(P<0.05).Acupuncture depth,acupuncture depth and needle thickness had little effect on TCM syndrome scores(P>0.05).Considering the combination of A2B2C1D1(30 min of needle retention,1 acupuncture per day,0.25 mm of needle selection,shallow spur),the improvement of TCM syndrome scores was the most significant.Regarding the influence of the score of neurological deficit score,the time of needle retention and the frequency of acupuncture had a greater influence on the score of neurological deficit(P<0.05).The frequency of acupuncture was more significant than the time of needle retention.The depth of acupuncture and the thickness of the needle The score of neurological deficit score was less(P>0.05).The combination of the relative excellent levels was A2B3C3D2 combination(30 min left,2 acupuncture per day,0.35 mm needle selection,moderate depth acupuncture).The improvement of neurological deficit score was the most significant.In terms of the impact of Fugl-Meyer motor function scores,four factors had an effect on limb motor function(P<0.05).The combination of the relative excellent level is A3B3C3D1(the needle retention time is 40 min,the daily acupuncture is 2 times,the needle body is 0.35 mm,and the acupuncture depth is selected as the shallow thorn).The combination of the limb movement function is most significant.In terms of the impact of Barthel scores,four factors had an effect on the recovery of patients’ daily living ability(P<0.05).When the factors are fixed,the combination of the relative excellent level is A3B3C1D2,considering the combination of A3B3C1D2(40 min left,2 times acupuncture per day,0.25 mm needle selection,moderate acupuncture depth)for the recovery of daily living ability of patients.The impact is most significant.Conclusion: Acupuncture needle retention time,Acupuncture frequency,needle thickness and Acupuncture depth are the main factors affecting the limb movement dysfunction of patients with Acupuncture treatment during stroke recovery.The most important factor for Acupuncture treatment of limb motor dysfunction during stroke recovery is the needle retention time.And the effect of selecting the needle retention time of 30 min or 40 min is better.Followed by the frequency of Acupuncture,treatment once a day or twice a day is relatively reasonable.The factors that have less influence are the thickness of the needle and the depth of the Acupuncture.In the group with obvious influence,the improvement of the movement function is obvious,the improvement of the daily life ability is obvious,and the depth of the Acupuncture is not recommended.Conclusion : The main influencing factor of acupuncture treatment of limb motor dysfunction in stroke recovery period is the time of needle retention,and the effect of needle retention time is 30min~40min.Followed by the frequency of acupuncture,treatment once a day or twice a day is relatively reasonable.The factors that have less influence are the thickness of the needle body and the depth of the needle acupuncture.The coarseness is obviously improved for the motor function,and the fineness of the daily life is improved obviously,and the depth of the acupuncture is not recommended for the deep thorn.
