| ObjectiveThe aim of the study was to investigate the correlation of serum Chemerin level in fatty pancreas(FP)patients with type 2 diabetes mellitus(T2DM)prevalence to provide a new perspective on the early prevention,diagnosis and treatment thereof in simple FP and FP with T2DM patients.Methods1.A total of 652 medical patients of the Affiliated Hospital of Yangzhou University in the province of Jiangsu in China who were diagnosed as fatty pancreas from January 2018 to June 2018,Each subject was assessed by using abdominal sonography to diagnose fatty pancreas.After the exclusion of 184 subjects based on incomplete information,a total of 468 subjects were selected as fatty pancreas group were divided into the simple fatty pancreas(groupA n=374)、fatty pancreas concurrent impaired fasting glucose(groupB1 n=44)and fatty pancreas concurrent type 2 diabetes mellitus(groupB2 n=50)according to blood glucose for further analysis.50 healthy subjects with normal blood glucose in the same period were included as control group N1(n=50).Another 30 subjects with simple type 2 diabetes mellitus in the same period were included as group N2(n=30).The sex,age,BMI blood pressure and blood biochemical indexes were compared among different groups.The changes of serum Chemerin level between different groups and correlation analysis of Chemerin and various indicators in fatty pancreas concurrent type 2 diabetes mellitus patients were also compared respectively.Finally,clinical and metabolic parameters were compared between groups,and their associations with fatty pancreas complicated by type 2 diabetes mellitus were analyzed by logistic regression.2.Randomly selected 88 medical patients from January 2018 to June 2018 in the Affiliated Hospital physical examination center of Yangzhou University according to the research conditions.Each subject was assessed by using abdominal sonography to diagnose fatty pancreas.a total of 88 subjects were divided into the control group(groupN N=12)and fatty pancreas group(groupP N=76),and P group was divided into the mild pancreatic steatosis group(groupPl N=7),the moderate pancreatic steatosis group(groupP2 N=54),the severe pancreatic steatosis(groupP3 N=15)according to the the degree of pancreatic steatosis.Observe the general clinical data,lipid levels,secretion of serum insulin and Chemerin between the four groups of participants and compare the differences between groups of islet function.Results1.The age,BMI,diastolic blood pressure,uric acid,WC,fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA-IR)and serum Chemerin level in group P were significantly higher and HDL,QUICKI indexes was lower than those in group N(P<0.05).2.The differences of age,BMI,systolic blood pressure,HDL,TG and uric acid in three groups of different levels of glucose tolerance in FP patients were statistically significant(P<0.05).QUICKI in groups FP was significantly lower than that in group Nl,and QUICKI in group B2 was obviously lower than that in group A,and the differences were statistically significant(P<0.05).There was no statistically difference in FINS,Chemerin and WC(P>0.05).3.Correlation analysis of Chemerin and various indicators:Spearman related analysis showed that serum Chemerin in FP with T2DM patients was positively correlated with HOMA-IR,TG,WTI and BMI(r=0.426~0.522,P<0.05).4.Logistic regression analysis:Logistic regression analysis showed that age,high triglycerides and high blood pressure were independent risk factors for FP complicated with type 2 diabetes.5.The differences of BMI,WC,FINS,HOMA-IR,QUICKI in four groups of different pancreatic steatosis patients were statistically significant(P<0.05).Chemerin in groups P3 were significantly higher than that in group N,P1 and P2,and the differences were statistically significant(P<0.05).ConclusionExpression level of serum Chemerin in fatty pancreas and type 2 diabetes mellitus patients were significantly higher than that in healthy controls,furthermore,serum Chemerin levels were closely related to the degree of pancreatic steatosis and it were also associated with insulin resistance in fatty pancreas and type 2 diabetes mellitus patients.Therefore,to test serum Chemerin level can help estimate the severity of fatty pancreas and identify the diabetes susceptible populations in fatty pancreas patients earlier. |
