| Objective: To investigate the related influencing factors of secondary vascular cognitive infarct(VCI)after acute cerebral infarction.Methods: A total of 109 patients with acute cerebral infarction who were diagnosed with acute cerebral infarction and who had excluded post-stroke depression(PSD)from September 2017 to December 2018 in First Affiliated Hospital of Wannan Medical College were selected.All patients used Montreal Cognition Assessment to evaluate the cognitive function level.According to the evaluation results,the patients were divided into VCI group and no VCI group,70 patients in VCI group and 39 patients without VCI group.The incidence of VCI after acute cerebral infarction and related factors of VCI were analyzed by statistical analysis.Results: 1.The incidence of vascular cognitive infarct in all patients was 64.22%.2.There was a statistically significant difference in the incidence of VCI between the infarction sites of acute cerebral infarction(χ2=18.3231,P=0.0026).3.Univariate analysis: In the general condition,the difference in the number of years of education between the two groups with and without VCI was statistically significant(P<0.001).Among the vascular risk factors,diabetes,leukoaraiosis,atrial fibrillation,coronary heart disease,carotid atherosclerosis,hyperuricemia were significantly different between the two groups(P <0.05).4.Multivariate analysis: years of education,diabetes,hyperuricemia,white matter lesions,atrial fibrillation,the above five variables were statistically significant(P<0.05),years of education(OR=0.810,95% confidence interval)0.671-0.978,P=0.0282),diabetes(OR=6.946,95% confidence interval 1.221-39.510,P=0.0289),hyperuricemia(OR=7.694,95% confidence interval 1.098-53.927,P=0.0400),White matter lesions(OR=85.468,95% confidence interval 7.743-943.451,P=0.0003),atrial fibrillation(OR=28.122,95% confidence interval 4.062-194.698,P=0.0007).Conclusion: 1.The incidence of VCI after all patients with acute cerebral infarction was 64.22%.2.The occurrence of VCI after acute cerebral infarction is related to the infarction site,and the lesions in the frontal,temporal and thalamic regions are more prone to VCI.3.The lower the education period of patients with acute cerebral infarction,the more likely VCI occurs;diabetes,leukoaraiosis,atrial fibrillation,coronary heart disease,carotid atherosclerosis,hyperuricemia may be related risk factors for VCI.4.Years of education,diabetes,leukoaraiosis,atrial fibrillation,and hyperuricemia are independent risk factors for VCI after acute cerebral infarction. |
