Objective: To analyze the changes of left ventricular configuration and function in patients with chronic heart failure(CHF)by velocity vector imaging(VVI)and speckle tracking imaging(STI),and to provide more Information guidance for diagnosis and treatment for patients with chronic heart failure.Methods: A total of 40 normal healthy controls and 59 NYHA functional grading methods were used to diagnose patients with grade III-IV heart failure and EF ? 50% by Simpson's biplane method(ie,heart failure with preserved ejection fraction,HFp EF).And 40 patients with NYHA cardiac function classification were diagnosed with grade III-IV of heart failure and EF <50%(HFr EF)by Simpson's biplane method.Two-dimensional echocardiography(2DE)was used to obtain left ventricular anteroposterior diameter(LV),interventricular septal thickness(IVSd),left ventricular posterior wall thickness(LVPWd),left ventricular end diastolic volume(LVEDV),and left ventricular systolic end volume(LVESV),left ventricular short axis shortening rate(FS),left ventricular ejection fraction(EF),left ventricular mass index(LV Mass Index,LVMI),calculation of maximal blood flow velocity of early mitral valve in each group of ventricular diastolic The ratio of the maximum blood flow velocity(E/A)to the mitral valve at the end of the ventricular end diastolic.Velocity vector imaging(VVI)technique was used to semi-quantitatively analyze the velocity vector plots of different segments of the left ventricular wall of the three groups of patients.Two-dimensional speckle tracking imaging(2DE-STI)technique measures left ventricular global longitudinal strain(GLS),global circumferential strain(GCS),and overall radial strain(GRS).Results: Comparison of conventional echocardiographic parameters: There were significant differences in LV between the three groups(P<0.05),and there was no significant difference between HFp EF and normal control group(P>0.05).There was no significant difference in IVSd between the three groups(P>0.05).There was no significant difference in LVPWd between the three groups,and the difference was not statistically significant(P>0.05).There was a difference in LVEDV between the three groups.The difference was statistically significant(P<0.05),and the difference between HFp EF and HFr EF was not statistically significant(P>0.05),which was higher than that of the normal control group.There were differences in LVESV between the three groups,and the difference was statistically significant.(P<0.05),and the difference between HFp EF and normal control group was not statistically significant(P>0.05),which was lower than that of HFr EF group.There was significant difference in FS between the three groups(P<0.05).and has a good correlation with EF;there is a difference in LVMI between the three groups,the difference is statistically significant(P <0.05),and the difference between HFp EF and HFr EF is statistically significant(P <0.05),both Normal control group increased;E/A difference between the three groups Statistically significant(P <0.05),and the difference between HFp EF HFr EF and not statistically significant(P> 0.05),less than 1.Compared with the normal control group,the speed vector curve of the left heart segment in the patients with chronic heart failure is disorderly.Comparison of two-dimensional speckle tracking imaging parameters: There were differences between GLS,GCS and GRS in the three groups,the difference was statistically significant(P<0.05),and the normal control group,HFp EF and HFr EF decreased in turn.Conclusion:The left ventricular local function is impaired in patients with HFp EF.The left ventricle is enlarged in patients with chronic heart failure,and the left ventricular global contraction and diastolic function are reduced to varying degrees.Velocity vector imaging combined with speckle tracking imaging technology can early detect the impairment of left ventricular function in patients with heart failure who retain ejection fraction,and provide assistance for clinical diagnosis and treatment.
