Chapter 1 Differential proteins expression in serum of patients with allergic rhinitis after rush immunotherapyObjectives:Rush immunotherapy for allergic rhinitis,as a method of subcutaneous injection of specific immunotherapy,which is rarely carried out at home and abroad,and its mechanism has not been elucidated.The expression of serum differential proteins plays an important role in the regulation of the mechanism of immunotherapy.However,the regulation of the differential expression of related proteins in the immune mechanism caused by short-term and high-dose administration of rush immunotherapy is still rare.In view of this,we used house dust mite vaccine to treat allergic rhinitis patients who were allergic to dust mite for 6 days of rush immunotherapy,and observed the expression of serum-related differential proteins.Methods:Ten healthy volunteers who were examined for non-allergic rhinitis in our hospital were selected as healthy control group(group A).Fifteen patients with allergic rhinitis diagnosed as dust mite allergy in our hospital were selected as the allergic rhinitis group(pre-rush immunotherapy group,group B).The 15 patients underwent rush immunotherapy for 6 days with full informed consent as the rush immunotherapy group(group C).We studied isobaric tags for relative and absolute quantitation(iTRAQ)proteomics studies on these three groups of subjects,and then determined the differential proteins we needed by two-dimensional gel electrophoresis and mass spectrometry.And then using the Cluster 3.0 software to classify the two dimensions of the expression quantity of the sample and the protein at the same time,and finally,using the Java Trewview software to generate a hierarchical clustering thermal map to test the rationality of the selected differential proteins.At the same time,the differential proteins that may play an important roleand the proteins closely related to the completion of the incremental phase of rush immunotherapy were further identified.Results:(1)The total number of proteins identified in this study was 893.(2)In this study,53 differentially expressed proteins were identified.Compared with healthy control group,24 proteins with P value of less than 0.05 and 1.2 times higher were found in allergic rhinitis group,which were closely related to the occurrence of allergic rhinitis,including 10 up-regulated proteins and 14down-regulated proteins.compared with the allergic rhinitis group,patients with allergic rhinitis underwent 6 days of rush immunotherapy.There were 29 proteins whose expression of proteins with a difference of P value of less than 0.05 and 1.2times higher,which were related to the effect after the incremental phase of rush immunotherapy was completed,of which 12 were up-regulated and 17 were down-regulated.Conclusions:(1)Compared with healthy control group,the expression of up-regulated allergic rhinitis group and the expression of down-regulated protein after 6 days of rush immunotherapy were CTHRC1,WDR89.Compared with healthy control group,AR group was down-regulated and the expression of up-regulated protein after 6 days of rush immunotherapy was CRP.(2)iTRAQ technique was used to obtain the differential expression profiles of serum proteome in allergic rhinitis group and after the incremental phase of rush immunotherapy.(3)In the incremental phase of rush immunotherapy,large doses of house dust mite allergen were given in a short period of time,which caused differential protein expression.Among them,the proteins with significant differential expression between groups were CTHRC1,WDR89 and CRP.Chapter 2 Identification and significance of CRP,CTHRC1 and WDR89 in serum of allergic rhinitis patients after rush immunotherapy Objectives:According to the above study,three differentially expressed proteins,CRP,CTHRC1 and WDR89,were selected to detect and identify their expression in serum of patients with allergic rhinitis and rush immunotherapy for 6 days.To analyze and explore the immunomodulatory effects and significance of these three differential proteins in rush immunotherapy of allergic rhinitis.Methods:The serum levels of CRP,CTHRC1 and WDR89 proteins of 11 healthy volunteers,40 patients with allergic rhinitis and 15 patients with rush immunotherapy for 6 days were tested by enzyme-linked immunosorbent assay.And the results were compared statistically respectively.Results:(1)The expression of CRP in serum: compared with the healthy control group,the expression level of CRP in AR group was not statistically significant(P > 0.05).Compared with AR group,the expression of CRP in RIT group was not statistically significant(P > 0.05).(2)The expression level of CTHRC1 in serum: compared with the healthy control group,the expression level of CTHRC1 in AR group was statistically significant(P < 0.0001).Compared with AR group,the expression of CTHRC1 in RIT group was statistically significant(P < 0.0001).(3)The expression level of WDR89 in serum: compared with healthy control group,the expression of WDR89 in AR group was statistically significant(P <0.0001).Compared with AR group,the expression of WDR89 in RIT group was statistically significant(P < 0.0001).Conclusions:In this part of the study,the differential expression of serum CTHRC1 andWDR89 were statistically significant in AR and RIT,while there was no significant difference in serum CRP expression level in AR and RIT.The results showed that serum protein CTHRC1 and WDR89 are closely related to the pathogenesis of AR,and played a role in the regulation of rush immunotherapy,while serum protein CRP has no significant effect in AR and RIT.The purpose of this study is to explore the role of differential protein expression in the immune mechanism of allergic rhinitis after rush immunotherapy and make a meaningful attempt.The mechanism of rush immunotherapy for allergic rhinitis is complicated.It needs further study to elucidate the immunomodulatory mechanism of its clinical application.
