| Objective By comparing with psychotherapy,the effectiveness and safety of integrated acupuncture regimen in the treatment of mild to moderate postpartum depressive disorder was observed.Methods 68 cases of postpartum mild to moderate depression were recruited in the acupuncture clinic or community of Guangdong Provincial Hospital of Traditional Chinese Medicine.Randomized digital method was divided into integrated acupuncture group and psychotherapy group.Integrate acupuncture group acupuncture points:Baihui,Yintang,Hegu(double),Taichong(double),mild reinforcing-attenuating,leaving needle for 30 minutes;fine moxibustion acupoint:four flowers,take a small amount of cashmere into a diameter Approximately 1-2mm,a height of about 3-5mm,a weight of about 20mg of compact Ai,placed on the acupuncture point with a line incense,when the patient feels skin burning,pain,then remove,each point of each moxibustion 2 strong;buried Acupuncture points:Xinshu(double),liver Yu(double),using a disposable sterile intradermal needle to penetrate the acupoints under the skin,the depth is about 3-5mm,the needle handle remains outside the skin,and then fixed with medical tape Ok,keep 2-3d.The psychotherapy group was psychologically intervened by the trained researchers,including postpartum health education,cognitive behavioral therapy,and supportive therapy.The treatment in both groups was performed twice a week for a total of 10 times for one course of treatment.The EPDS score,HAMD score,SDS score,and the overall clinical efficacy after treatment and follow-up were observed before treatment,after treatment,and 1 month follow-up.The safety of integrated acupuncture regimen for postpartum depression was evaluated. Statistical analysis was performed using SPSS 20.0 software,and the difference was statistically significant at P<0.05. The measurement data conforming to the normal distribution is expressed by the mean ± standard deviation(),and the normal distribution test is performed by the ShaPiro-Wilk test.The normal measurement of the data between the two groups is compared,and the homogeneity F test is first performed.If the variance is uniform,Using the t test,if the variance is not uniform,the t’ test is used.The paired t test is used for the self-control before and after the normal measurement data.The non-normal measurement data is expressed by the median and interquartile range[M(OR)].The count data were expressed by frequency,composition ratio and percentage.The non-normal measurement data between the two groups were analyzed by Mann-Whitney U test.Wilcoxon Z test was used for self-control before and after treatment.The four-table data is based on the x2 test or the Fisher test. The safety analysis uses descriptive statistics,including the incidence of adverse events and their specific details;clearly states the reasons for the shedding of the cases,and calculates and describes the rate of shedding of each group.Results A total of 65 postpartum mild-to-moderate depression subjects were included in the study,including 32 in the acupuncture group and 33 in the psychotherapy group.The age,duration,occupation,education level,TCM classification,mode of delivery,infant gender,and pre-treatment scores of the two groups were tested.The difference was not statistically significant(P>0.05),and the groups were comparable. 1.EPDS scale results. There was no significant difference in the EPDS scores between the two groups after treatment and 1 month follow-up(P>0.05),suggesting that the two groups were equally effective.In the integrated acupuncture group,the EPDS scores in the same group after treatment and at 1 month follow-up were compared with those before treatment(Z=-4.726,P=0.000;Z=-4.704,P=0.000),and the differences were statistically significant.(P<0.05);in the psychotherapy group,the EPDS scores of the same group after treatment and 1 month of follow-up were compared with those before treatment(Z=-4.657,P=0.000;Z=-4.480,P=0.000),The difference was statistically significant(P<0.05),indicating that both acupuncture and psychotherapy can reduce the score of EPDS. The overall trend of EPDS,after treatment and at 1 month of follow-up,the EPDS scores of the two groups were lower than before treatment,indicating that regardless of which intervention can alleviate the patient’ s depressive symptoms,but during the follow-up of 1 month The EPDS scores of the two groups showed a certain degree of rebound after treatment,suggesting that after the end of the treatment,both treatments gradually decreased with time. 2.HAMD scale results. There was no significant difference in the HAMD scores between the two groups after treatment and 1 month follow-up(P>0.05),suggesting that the efficacy of the two groups was comparable.In the integrated acupuncture group,the HAMD scores of the same group after treatment and 1 month of follow-up were compared with those before treatment(Z=-4.941,P=0.000;Z=-4.943,P=0.000),and the differences were statistically significant.(P<0.05);in the psychotherapy group,the HAMD scores of the same group after treatment and 1 month of follow-up were compared with those before treatment(Z=-4.928,P=0.000;Z=-4.976,P=0.000),The difference was statistically significant(P<0.05),indicating that both acupuncture and psychotherapy can reduce the HAMD score. The overall trend of HAMD,after treatment and 1 month of follow-up,the HAMD scores of the two groups were lower than before treatment,indicating that regardless of which intervention can alleviate the patient’s depressive symptoms,but during the follow-up of 1 month The HAMD scores of the two groups showed a certain degree of rebound after treatment,suggesting that after the end of the treatment,both treatments gradually decreased with time. 3.SDS scale results There was no significant difference in SDS scores between the two groups after treatment and 1 month follow-up(P>0.05),suggesting that the two groups were equally effective.In the integrated acupuncture group,the SDS scores of the same group after treatment and 1 month of follow-up were compared with those before treatment(Z=-4.825,P=0.000;Z=-4.531,P=0.000),and the differences were statistically significant.(P<0.05);In the psychotherapy group,the SDS scores were compared with those before treatment(Z=-4.811,P=0.000;Z=-3.426,P=0.001)after treatment in the same group and at 1 month follow-up.The difference was statistically significant(P<0.05),indicating that both acupuncture and psychotherapy can reduce the SDS score. The overall trend of SDS,after treatment and at 1 month of follow-up,the SDS scores of the two groups were lower than before treatment,indicating that regardless of which intervention can alleviate the patient’s depressive symptoms,but during the follow-up of 1 month The SDS scores of the two groups showed a certain degree of rebound after treatment,suggesting that after the end of the treatment,both treatments gradually decreased with time. 4.Overall efficiency comparison At the end of treatment and 1 month of follow-up,the total effective rate of the integrated acupuncture group was 87.5%,78.1%,and the total effective rate of the psychotherapy group was 72.8%,60.6%.The overall effective rate of the two groups was analyzed,and the difference was not statistical.Academic significance(P>0.05). 5.Security analysis A total of 65 subjects were enrolled in the study,and 2 adverse events occurred.After simple treatment,the symptoms disappeared and the overall safety was good.Conclusion Integrating acupuncture and psychotherapy can improve mild to moderate postpartum depressive symptoms,and its efficacy is comparable. |
