Objective: The clinical data of patients with high-grade gliomas were analyzed,and the expression of immuno-associated protein IDO1 was detected by the immunohistochemical method in postoperative pathological tissues.In order to explore the expression of IDO1 protein in high-grade gliomas and its correlation with prognosis.Methods: From July 2008 to May 2016,54 patients diagnosed with highgrade gliomas who underwent surgery-based comprehensive treatment in Hunan tumor hospital were recruited,which including 42 patients with WHO III grade and 12 patients with WHO IV grade.The expression of IDO1 protein in postoperative pathological tissues of high-grade gliomas was detected by immunohistochemistry.SPSS 20.0 statistical software was used to analyze the data,and the Pearson Chi-square test was used to analyze the relationship between the expression of IDO1 protein and clinical characteristic variables.Survival analysis was performed by the Kaplan-Meier method,and univariate analysis was performed by the Log-Rank test and multivariate analysis was performed by the Cox regression model.All of these were designed to determine the correlation between the expression of IDO1 protein and the prognosis of patients with high-grade gliomas.Result: During the follow-up time of 3-120(median 34.00)months,40patients(74.07%)died and 14 patients(25.93%)survived.The median overall survival time(MOST)of the patients was 31.00(95% CI 10.49 to 51.51)months,the median progression-free survival time(MPFS)of the patients was17.00(95% CI 11.86 to 22.14)months.The positive staining reaction of IDO1 protein mainly occurred in the cytoplasm,and the high expression rate of IDO1 protein was 35.19%(19/54).Patients with high-grade gliomas with low expression of IDO1 protein had longer MOST than those with high expression of IDO1 protein(43.00 vs 18.00)(P=0.041,P<0.05).Patients with high-grade gliomas with low expression of IDO1 protein had longer MPFS than those with high expression of IDO1 protein(31.00 vs 9.00)(P=0.001,P<0.05).Univariate analysis with the Log-Rank test showed that the factors associated with the prognosis of patients with high-grade gliomas included the pathological grade of tumor,the degree of resection,and expression of IDO1protein(P < 0.05).Multivariate analysis with the Cox regression model suggested that the pathological grade of tumor and the degree of resection were independent prognostic factors for patients with high-grade glioma(P <0.05).Conclusion: This study has shown that the overall survival time,progression-free survival time of patients with high-grade glioma were improved after comprehensive treatment.Factors affecting the prognosis of patients include the pathological grade of tumor,the degree of resection,and expression of IDO1 protein.Patients with high-grade gliomas with high expression of IDO1 protein have a poor prognosis.Therefore,IDO1 as a prognostic marker and potential immunotherapy target for high-grade gliomas should be further investigated.
