ObjectiveThrough clinical retrospective comparative study,the efficacy and safety of occipital exercise with percutaneous kyphoplasty on the treatment of osteoporotic thoracolumbar vertebral compression fractures were analyzed,and the effects of this treatment on the reduction height,vertebral function,life quality and incidence of re-fracture of adjacent vertebral bodies after kyphoplasty were discussed.The theory of integrated traditional Chinese and Western medicine "laying equal stress on tendons and bones(unification of tendons and bones’ structure and function),combination of motion and static(unification of active and passive activities),combination of internal and external treatment(unification of local and overall functional recovery),doctor-patient cooperation(doctor-patient cooperation must be throughout the whole rehabilitation process)" on the treatment of osteoporotic thoracolumbar vertebral compression fractures by occipital exercise with percutaneous kyphoplasty were explored.MethodThe 68 patients with osteoporotic thoracolumbar vertebral compression fracture who met the research criteria were divided into experimental group and control group.There were 34 cases in experimental group and 34 cases in control group.34 cases in experimental group were treated With pillow exercises with percutaneous kyphoplasty,while 34 cases in control group were treated with percutaneous kyphoplasty alone.The retrospective study was conducted.The degree of lumbar back pain,Oswetry dysfunction index(ODI)score,ADL score,vertebral height loss rate and adjacent vertebral re-fracture rate in two groups were evaluated,and data before treatment(baseline),before operation,3 days,1 month,3 months and 6 months after operation were collected and analyzed.The CRF table was used to collect,save and manage the relevant raw data.The research work was carried out strictly by the design scheme to ensure the standardization of data measurement and collection,and timeliness,authenticity and integrity of data was regularly checked and monitored.The statistical software SPSS25.0 was used to process and analyze the data by bilateral tests.P<0.05 showed that the difference was statistically significant.ResultRetrospective study of 68 cases in Beijing Pingli Hospital were selected strictly in accordance with the design standards of scientific research to meet the selection and exclusion criteria,including 34 cases in experimental group and 34 cases in control group.The general medical records and baselines in experimental group and control group were comparable(P>0.05).In terms of overall curative effect.Chi-square test showed that proportion of good curative effect in experimental group(100.00%)was higher than that in control group(88.24%),and the difference was statistically significant(P<0.05).In terms of imaging,independent sample T test showed that there was no significant difference on vertebral height loss rate between experimental group and control group in 3 days after operation(P>0.05).The vertebral height loss rate between experimental group and control group was significantly different in 3 months and 6 months after operation(P<0.05).The Cobb angle of kyphosis between experimental group and control group at 3 months after operation had statistical difference(P<0.05)In terms of pain,independent sample T test showed that there was no significant difference in the VAS score of back pain between experimental group and control group at 3 days after operation(P>0.05),and there was significant difference on the VAS score of back pain between experimental group and control group at 1 month.3 months and 6 months after operation(P<0.05).In terms of lumbar function,independent sample T test showed that there were significant differences on lumbar Oswetry dysfunction index between experimental group and control group after operation(P<0.05).Chi-square test showed that there were significant differences on rate of adjacent vertebral re-fracture between experimental group and control group within 6 months after operation(P<0.05).There were significant differences on ADL scores between experimental group and control group within 6 months after operation(P<0.05).Conclusion1 The pillow exercises with percutaneous kyphoplasty can effectively promote the recovery of kyphosis Cobb angle and vertebral height,and prevent the loss of vertebral height and the occurrence of adjacent vertebral re-fracture.2 The pillow exercises with percutaneous kyphoplasty can alleviate the low back pain,and enhance the lumbar function and life quality.3 The pillow exercises with percutaneous kyphoplasty can effectively shorten the time of patients lying in bed of patients with osteoporotic thoracolumbar vertebral compression fracture.Thedirection of fracture treatment is the combination of traditional Chinese and Western medicine,evaluation and exercise,doctor-patient cooperation,and laying equal stress on tendons and bones(unification of structure and function of paravertebral muscles,tendons and bones)and combination of motion and static(unification of active exercises and passive pillow activities),internal and external treatment(local kyphoplasty and overall functional recovery of pillow exercises),doctor-patient cooperation(doctor-patient cooperation must run through the whole process of fracture treatment)are the principles of fracture treatment.The criteria for evaluating fracture-selection of pillow exercises-doctor-patient cooperative reduction-kyphoplasty-standardized anti-osteoporosis-functional exercise of lumbar and dorsal muscles has achieved.In this study,the method has remarkable clinical efficacy on osteoporotic thoracolumbar vertebral compression fractures,which is worthy of promotion and application.
