Objective Using proton magnetic resonance spectroscopy(1H-MRS)technology to analyse the characteristics of changes of H-MRS biochemical metabolite parameters in hippocampus in patients with vascular cognitive impairment no dementia(VCIND)and to discuss the relationship with Cognitive Assessment(MoCA)examination in VCIND.Materials and methods A total of 22 VCIND(VCIND group)patients and 22 matched conditions volunteers(NC group)were enrolled in this study.All participants were measured bilateral hippocampal volume and examined with brain MRI and 1H-MRS in the bilateral hippocampus.For each one,N-acetyl aspartate(NAA),choline compound(Cho)and crestine compound(Cr)were calculated,and the correlation between the ratio of NAA/Cr,Cho/Cr and MoCA scores was analysed in VCIND groups.All subjects were right-handed.Results 1.There was no significant difference in hippocampal volume between VCIND group and NC group(P>0.05);2.Compared with normal control(NC)group,the ratios of NAA/Cr in bilateral hippocampus were significantly decreased in VCIND group,and the difference was statistically significant(Left,F=3.13,t=-4.39,P=0.00,P<0.05;Right,F=1.67,t=-3.18,P=0.01,P<0.05).There was no significant difference in Cho/Cr in bilateral hippocampus between the groups(P>0.05);3.The ratios of NAA/Cr in the left hippocampus were lower than those in the right hippocampus in VCIND group,and the difference was statistically significant(t=-3.01,P=0.01,P<0.05),and the ratios of NAA/Cr in bilateral hippocampus were no significant difference in NC group(P>0.05).The ratios of Cho/Cr in bilateral hippocampus were no significant difference in both groups(P>0.05);4.The ratios of NAA/Cr in bilateral hippocampus in VCIND group were significantly correlated with MoCA score(Left,r=0.77,P=0.00,P<0.05;Right,r=0.61,P=0.00,P<0.05),and there was no significant difference between the ratios of Cho/Cr in bilateral hippocampus and MoCA score(P>0.05).Conclusion 1.Changes in 1H-MRS metabolites in the hippocampus may indicate the existence of hippocampal neuron damage before the occurrence of morphological changes in the hippocampus;2.1H-MRS NAA/Cr can reflect the cognitive function changes of patients to a certain extent,which is helpful for the early diagnosis of VCIND.
