Objective:Through the preoperative examination of anal fistula by transperineal ultrasound combined with transrectal ultrasound and a routine MRI imaging,the accuracy and application value of the two examination methods in the preoperative diagnosis of anal fistula can be analyzed.Methods:All patients were treated with transperineal ultrasound combined with transrectal ultrasonography and a routine MRI examination for preoperative diagnosis of anal fistula according to the retrospective analysis of 81 patients with anal fistula surgery in the Yancheng TCM hospital.By comparing the analysis with the diagnosis results of the clinician's operation,the number of main fistulas,branches,internal orifices,external orifices,and the classification of anal fistula of the two kinds of diagnostic methods can be achieved.Then we count up the coincidence rate of the surgical results and the analysis of the advantages and disadvantages of transrectal ultrasound combined with transperineal ultrasound and conventional MRI examination.At last,we will get a conclusion of the more convenient and effective method for the diagnosis of anal fistula.Results:(1)For the exploration of the main fistula,the detection rate of transperineal ultrasound combined with transrectal ultrasound was 94.49%,the detection rate of conventional MRI examination was 96.85%.There was no statistical difference(P>0.05).(2)For the examination of branch fistula,the detection rate of transperineal ultrasound combined with transrectal ultrasound was 95.24%,the detection rate of conventional MRI examination was 98.41%.There was no statistical difference(P>0.05).(3)For the exploration of internal port,the detection rate of transperineal ultrasound combined with transrectal ultrasound was 95.97%,the detection rate of conventional MRI examination was 98.39%.There was no statistical difference(P>0.05).(4)For the exploration of external port,the detection rate of transperineal ultrasound combined with transrectal ultrasound was 99.23%,the detection rate of conventional MRI examination was 98.46%.There was no statistical difference(P>0.05).(5)According to the relationship between the main fistula of the anal fistula and the anal sphincter,we could get the Parks classification:First,the dection rate of the type of intersphincter and transsphincter through the two joint ultrasound diagnosis was almost the same as conventional MRI examination method,and there was no statistical difference in the two groups(P>0.05);Second,the diagnosis rate of the type of extrasphincter and suprasphincter through the two joint ultrasound diagnosis was lower(P<0.05).Conclusion:Thereis no marked statistical difference between the transperineal ultrasound combined with transrectal biplane ultrasound and conventional MRI in the preoperative diagnosis of anal fistula.The combination of transperineal ultrasound and transrectal biplane ultrasound in the diagnosis of anal fistula has higher accuracy,better operability and lower cost.Less contraindication than MR examination,it can get widely used and applied in clinical practice.It can be used as one of the simple and effective examinations for anal fistula and offer clinic more options.
