Study On Plasma LncRNA-NEAT1 Expression In Patients With Coronary Artery Disease And Its Clinical Value

Background and Objective:Coronary artery disease(CAD)is one of the most important cardiovascular diseases,early identification and intervention is the important way to effectively control the disease and reduce its mortality.Studies have shown that long non-coding RNA(LncRNA)is involved in the occurrence and development of CAD through different pathways and may play an important role in it.We compared the expression of LncRNA-NEAT1 in plasma of coronary artery negative group and patients with different clinical types of CAD.Furthermore,to explore the feasibility and value of plasma NEAT1 as a potential biomarker for predicting CAD.Method:1.From January 2018 to October 2018,158 patients with chest tightness or chest pain as the main complaint were hospitalized in the Department of Cardiology,the first affiliated Hospital of Zhejiang University Medical College and finally confirmed by percutaneous coronary angiography.The expressions of plasma NEAT1 were detected by qRT-PCR.2.Compared the difference of plasma NEAT1 expression between coronary artery negative group and CAD group,then further analyzed the difference between stable coronary artery disease(SCAD)and unstable angina(UA)in CAD group.3.Analysed the correlation between plasma NEAT1 and each clinical feature of CAD.4.Univariate and multivariate logistic regression analysis were used to explore the independent influencing factors for the diagnosis of CAD and UA type.The feasibility of plasma NEAT1 as a potential biomarker for predicting CAD and UA was evaluated by ROC curve and AUC,then combined with various risk factors of CAD to improve the accuracy of prediction.Results:1.The NEAT1 was stable and measurable in plasma,the expression level of plasma NEAT1 in CAD was significantly higher than that in coronary artery negative group(P<0.001),which was especially higher in UA group(P<0.001),but there was nosignificant difference between coronary artery negative group and SCAD group(P>0.05).2.The expression level of plasma NEAT1 was significantly correlated with the clinical classification of CAD(P<0.001),but not with gender,age,hypertension or smoking.3.Univariate and multivariate logistic regression analysis showed that plasma NEAT1 expression was positively correlated with the diagnosis of CAD,and further had a similar performance in the diagnosis of UA.ROC analysis showed that the AUC of CAD predicted by single factor of plasma NEAT 1 was 0.690(95%CI?0.607-0.773,P<0.001),and the AUC of combined multivariate prediction of CAD was 0.786(95%CI=0.717-0.856,P<0.001).The AUC of UA predicted by single factor of plasma NEAT1 was 0.763(95%CI?0.672-0.854,P<0.001),and the AUC of combined multivariate prediction of UA was 0.808(95%CI=0.729-0.887,P<0.001).Conclusions:The expression level of plasma NEAT1 was significantly different between coronary artery negative group and CAD group,which was independently positive correlated with CAD,and further independently positive correlated with the type of UA in CAD.It is suggested that plasma NEAT1 may be helpful to improve the diagnosis of CAD,especially to predict the development of coronary atherosclerotic plaques.
