Objective:To explore the appropriate timing and methods of plastic surgical treatment of scalp and facial infantile hemangioms.Methods:Between Janurary 2012 to June 2018,20 patients with scalp and facial infantile hemangiomas were treated,and the clinical data were retrospectively analyzed.There were 10 males and 10 females,ages range from 3M to 6Y.The Scalp was involved in 3 cases,8 cases in face,3 cases in lips,1 case in ear,and 5 cases involved in both face and scalp.The area of lesion range from 0.5cm*lcm to 5cm*5cm.According to the different anatomical spots,the appropriate incision was designed individually according to the anatomical location of the hemangiomas,lesions can be removed with lenticular excision and linear closure or circular excision and purse-string closure or lacal skin flap transfer and repair.Under the goal of resection the tumor completely,we try to minimize the sacrs.One stage excision was performed in all cases.Result:From February 2012 to December 2018,we followed the postoperative results of 15 patients through phone calls and social media,the follow time from 1 week to 12 months after operation,and the average following time was 1 month.2 of the cases have obvious scars,other patients are satisfied with the surgical outcomes.Conclusion:Although surgical treatment is the second-line treatment for infantile hemangiomas now,surgery remains an important modality,to plastic surgeon,in the treatment of IHs.Therefore,those who have no response to beta blocker therapy(ie,propranolol)and IHs that are unlikely to involute compeletely,and IHs are in the loacations which can be removed very easily,we should preform surgical correction as early as possible to achieve a good appearance and function improvement and less scar without waiting for its involution.
