Application Of The "Kite" Subcutaneous Pedicle Flap In Defect Repair After Facial Skin Tumor Resection

ObjectiveThe "Kite" subcutaneous pedicle flap was used to repair the secondary defect after facial skin tumor resection.The clinical effect of this kind of skin flap repairing skin defect was discussed,which provided a basis for the clinical application of repairing facial skin defect.MethodsFrom September 2015 to December 2017,106 patients with facial skin tumors from 21 to 83 years old who underwent surgery in our department were selected as the skin tumor group.According to the shape of the skin lesion,the circle is cut off,and a triangular"kite"flap pedicled with the subcutaneous tissue directly below or on both sides of the flap is designed and made on the side adjacent to the defect,and the flap is advanced and filled into the defect area.Defect.The diameter of the defect and the length of the flap were measured intraoperatively.Six months after surgery,the scar was scored according to the Vancouver Scar Scale compared with the scar scores of 92 patients who underwent subcutaneous tumor resection in our department(the subcutaneous mass group),the subcutaneous mass group was done.Straight incision(no missing skin),resection of the mass and suture.The scores of the two groups of scars,the diameter of the defect measured in the skin tumor group,and the length of the flap were statistically analyzed,and the follow-up of the skin tumor group was recorded.Result1.A total of 106 skin flaps were enrolled in 106 skin tumors,all of which survived without"cat ear"malformation.No follow-up of tumor metastasis and recurrence was observed during 6 months to 2.5 years of follow-up.The incisions in the skin tumor group and the subcutaneous mass group were healed by first intention.The mean scores of the scars in the two groups were 1.25 points and 1.14 points,respectively,and the difference was not statistically significant(P>0.05).2.The"Kite"subcutaneous pedicle flap can repair the area of facial defects are:eyebrow bow is(1.27~3.91)cm~2;lower jaw is(0.54~1.25)cm~2;nasal sulcus is(0.69~4.91)cm~2;nasal lip The groove is(0.50~1.58)cm~2;the cheek is(1.91~9.62)cm~2;the anterior ear area is(1.35~4.95)cm~2.3.The"Kite"subcutaneous pedicle flap length and defect diameter ratio is:eyebrow bow is 1.76~2.26,lower jaw is 1.51~2.03,nasal sulcus is 1.62~2.5,nasolabial fold is 1.50~1.98,cheek is 1.80~2.34,the area in front of the ear is 1.51~2.03.Conclusions1.The "Kite" subcutaneous pedicle flap is used to repair facial skin defects.The scars left by the left scar and the straight cut skin(no missing skin)and sutures are not significant.The scars are small,the flaps are easy to take,and the blood is transported.Reliable,it is a reliable repair option for skin defects that are not suitable for direct sutured skin defects due to excessive tension,or that directly cause sutures to cause deformity of facial organs.2.According to the results,(0.50~9.62)cm~2facial round skin defect,using the"Kite"subcutaneous pedicle flap repair,can achieve satisfactory results.3.The"Kite"subcutaneous pedicle flap length is designed to be 1.5 to 2.5 times the diameter of the defect is more reasonable,the flap can not only completely repair the defect,the remaining scar is small.
