Professor Lu Chuanjian's Law On The Compatibility Of External Bath Formula In The Treatment Of Psoriasis Based On Data Mining

ObjectiveTo explore medication laws in Chinese medicine external treatment for psoriasis vulgaris by Professor Lu Chuanjian.To provide theoretical basis and ideas for the clinical application of psoriasis vulgaris by Summarizing the core side and analyzing the rationality of the party.MethodsCollected the cases of psoriasis vulgaris who visited the clinic and of Professor Lu Chuanjian of Guangdong Provincial Hospital of Traditional Chinese Medicine from 2010.01.01 to 2018.5.31 that met the inclusion criteria.The patient's name,gender,age,severity of illness,stage of disease,symptoms of skin lesions after medication,and prescriptions for Chinese medicine were entered into Epidata 3.1 software.WPS Office Excel 2016 was exported to establish a database and standardize data after checking.SPSS Statistics 22.0 was used for frequency statistics,principal component factor analysis and K-means cluster analysis.SPSS Clementine 12.0 was used for Apriori association rules.ResultsA total of 203 psoriasis vulgaris including 109 males and 94 females,with an average of 38.1 years,were included in the study.According to the severity of psoriasis,25 patients with mild psoriasis,79 patients with moderate psoriasis,59 patients with moderate to severe disease,and 40 patients with severe disease.According to the psoriasis stage,there were 147 patients with advanced disease,55 patients with stable phase,and 1 patient with remission.In terms of symptom improvement,the erythema remission rate was 91.1%,the scaly improvement rate was 87.2%,the infiltration improvement rate was 85.2%,the range reduction improvement rate was 74.4%.The head remission rate was 35.8%in the lesion area,the remission rate was 52.1%,the upper limb remission rate was 63%,the lower limb remission rate was 73.6%.The skin itching improvement rate was 71%,the dry improvement rate was 41.7%,the pain improvement rate was 12.5%,the burning improvement rate was 33.3%.,and the swelling improvement rate was 44.4%.Among the 203 prescriptions included in the study,a total of 43 flavors of single-flavored Chinese medicine were used,involving a total of 1812 times.The top ten drugs with the highest frequency of use were polygonatum(197),Rhizoma Polygonati(194),cooked rehmannia(190),dictamnus bark(189),Chinese angelica(185),smooth greenbrier root(183),cnidium seed(167),alum(135),paniculate cynanchum root(101),kochia fruit(56).The total frequency of the first 21 traditional Chinese medicines with high frequency(>5 times)is 1768 times,which accounts for 97.6%of the total frequency of the drugs.The most frequently used high-frequency drugs are tonic and heat-clearing drugs.Most of them are blood-supplementing medicinal and nourish yin medicine.More heat-clearing dampness-drying medicinal,heat-clearing toxin-resolving medicinal and heat-clearing blood-cooling medicinal are used in heat-clearing medicine.Apriori association rules was performed on 21 high-frequency Chinese medicines in 203 prescriptions.The minimum support was 10,the minimum confidence was 80,and the maximum number of front items was set to 1.46 pairs of strong correlation rules were obtained.The support degree was 10,the minimum confidence was 80,and the maximum number of front items was set to 2,which could obtain 335 sets of drug associations with strong correlation association rules;each of the top 20 rules for lifting was selected.The KMO value of the factor model was 0.807,which was greater than 0.60,and passed the Bartlett sphericity test with a significance level less than 0.05.These high-frequency drugs were suitable as a variable for factor analysis,and the principal component analysis method was used to extract the common factor variance.The cumulative contribution rate of eigenvalues>1 was close to 70%,and 5 common factors were extracted.It is considered that Professor Lu Chuanjian's K-means cluster analysis for the treatment of psoriasis vulgaris is suitable for five categories,and one core party and four new groups are obtained combined with clinical practice.ConclusionsThe external bath formula is mainly used for the treatment of patients with advanced and moderate to severe psoriasis vulgaris,and the clinical effect is improving the symptoms of psoriatic lesions and relieving pruritus.The drug category reuses nourishing Yin and moistening dryness drugs,heat-clearing toxin-resolving medicinal,clear heat and dispel dampness drugs.The association rules of the drug on the drug group,cluster analysis of the common factor efficacy is mainly for clear heat and resolve toxin,nourishing Yin and moistening dryness,dry dampness(percolate dampness)and relieve itching.It is indicated that dry-pathogen,heat evil and wet evil are important pathological factors of this disease.There is damp heat inside,there is dryness outside,wet and hot glue,dampness and mutuality,so psoriasis is difficult to heal.Combined with clinical analysis,cluster analysis showed that the sovereign was cortex dictamni,the minister was ang smooth greenbrier root and cnidium seed.the assistant was Rhizoma Polygonati,cooked rehmannia,Chinese angelica,and the envoy was pomegranate bark.The formula played an important role in mild skin,moistening dryness,fortifying the spleen,dispeling dampness and antipruritic.
