ObjectThis study by detecting serum immunoglobulin IgA,IgG,IgM,IgE,peripheral blood T cell sulogroup CD4+,CD8+,CD4+/CD8+ of copd stabilization patients with lung spleen-deficiency,who accept self-blood point injection therapy in combination with basic treatment of western medicine in the treatment before and 3 months after treatment,and combined with pulmonary function(FEV1,FEVC,FEV1/FVC),6MWD,mMRC indicators,syndrome points,so as to learn clinical effects and the changes of immune function of patients with copd,which compared with only accepted basic therapy of western medicine in the treatment,and so as to make comprehensive clinical diagnosis and treatment of copd.MethodsA prospective randomized controlled observational study was conducted.From the outpatient department and inpatient department of the first affiliated hospital of guangzhou university of traditional Chinese medicine,68 cases of COPD patients with stable lung-spleen qi deficiency were included.The patients were randomly divided into treatment group(n=34)and control group(n=34).The control group was given basic treatment of western medicine(refer to GOLD 2018 "guidelines for the diagnosis and treatment of chronic obstructive pulmonary disease"),and the treatment group was given self-blood point injection therapy on the basis of basic treatment of western medicine,one course of treatment was given every 5 times,injection was given every other day,and a pair of points of the same name were selected each time(5 times were all different points).The second course of treatment was conducted 10 days after the end of the first course of treatment,and the third course of treatment was conducted every 10 days.A total of 3 courses of treatment were conducted,15 times in total,for a total of 50 days.The changes of immune function and scale scores in the two groups were observed.Results1.There was no statistical difference between the two groups in age,gender,lung function grading and other general conditions,as well as lung function,immune index,6MWD,mMRC before treatment,which was comparable.2.The comparison of lung function between the two groups before and after treatment,FEV1 and FVC were significantly improved after the treatment of acupoint self-blood injection therapy,and the differences were statistically significant(P<0.05),indicating that it could improve lung function to a certain extent.3.The comparison of immune indexes between the two groups before and after treatment showed that p<0.05,suggesting that acupoint injection therapy combined with basic western medicine may better improve the immune function of patients than the basic western medicine treatment alone.4.Comparison of 6WMD between the two groups before treatment,30 days after treatment and after treatment,and comparison of 6WMD before and after treatment in the control group showed no statistical significance(p>0.05),while comparison of 6WMD before and after treatment in the treatment group showed significant difference(p<0.05).It indicates that compared with the basic treatment of western medicine alone,the effect of self-blood point injection combined with basic treatment of western medicine for COPD patients with lung-spleen qi deficiency is better to increase 6MWD.5.Intergroup comparison of mMRC between the two groups,and intra-group comparison of mMRC in the treatment group showed significant differences(p<0.05),suggesting that acupoint injection therapy combined with western medicine for COPD patients with lung-spleen qi deficiency can reduce mMRC index,while western medicine for basic treatment can not significantly reduce mMRC index.6.Two groups after treatment symptoms integral compared between groups,as well as in the two groups before and after treatment in group comparison results have significant difference(p<0.05),suggesting the blood point injection therapy in combination with the basic treatment of western medicine and western medicine foundation treatment all can improve symptoms in patients with COPD stabilization lung-spleen qi deficiency integral,but the blood point injection therapy in combination with western medicine foundation treatment to improve symptom integral effect is better.7.There was no significant difference in the total effective rate between the two groups after treatment(p>0.05);However,the p values of cough,sputum cough,wheezing,shortness of breath,stuffiness,abdominal distension and sweating after treatment were all less than 0.05,indicating that the improvement of the above symptoms in the treatment group was better than that in the control group.ConclusionSelf-blood point injection therapy in combination with western medicine foundation treatment can improve lung function in patients with COPD stabilization lung spleen-deficiency,delay lung function decline,increased significantly in patients with T cell subgroup CD4+,CD8+,CD4+/CD8 and immunoglobulin IgA,IgG,IgM,6 WMD,reduce IgE,mMRC index reduction and syndrome points,and combination therapy combination than western medicine has obvious advantages in foundation treatment.
