Objective:This essay aims to investigate the white matter microstructure and its correlation with cognitive function in first-episode depression,so as to further understand the neuropathological mechanism of the onset of depression and cognitive impairment.P rovide an theoretical support for early diagnosis and intervention in depression.Methods:Selected from March 2018 to December 2018 in Guangdong 999 Brain Hospital first-episode for the survey.Using the repeatable battery for the assessment of neuropsychological status(RBANS)to evaluate their cognitive function of the patients.Then in turn for diffusion tensor imaging(DTI)to scan their white matter integrity,and using statistical methods to analyze the correlation between cognitive function and diffusion tensor imaging indicators.Results:1.Results of cognitive function assessmentThis study collected 45 patients with first-episode depression.The RBANS results showed that the scores of the patients’ RBANS and items(immediate memory,visual span,speech function,attention and delayed memory)were significantly lower than those of the control group(p<0.05).Correlation analysis results showed that the RBANS total score and instant memory scores of patients with different education levels were statistically significant(p<0.05),while the differences in visual breadth,speech function,attention and delayed memory scores were not statistically significant(p>0.05).The scores of RBANS,immediate memory,verbal function,attention and delayed memory of patients with different clinical types were statistically significant(p<0.05),while the scores of visual span were not statistically significant(p>0.05).2,Diffusion tensor imaging analysis resultsThis study collected 16 patients with first-episode depression.DTI analysis results show that patients in left thalamus radiation at the front,the left and right cingulate gyrus,left hippocampus and corpus callosum radiation occipital,on the right side of the pillow,the left and the right side of the beam under longitudinal fasciculus,on the left side of the longitudinal fasciculus,hooked on the left,the right side of the beam on the longitudinal fasciculus temporal FA value contrast control group differences statistically significant(p<0.05),of which the left thalamic radiation at the front,on the left side of the longitudinal fasciculus,hooked on the left beam FA value less than.The results of correlation analysis showed no correlation between age,gender,course of disease,clinical type,and cognitive function and FA values of left anterior thalamic radiation,left upper longitudinal tract,and left uncinate tract.(p>0.05)Conclusion:1.First-episode depression have multiple cognitive impairment,including immediate memory,visual span,speech function,attention,delayed memory,and those with psychiatric symptoms have more severe cognitive impairment.2.White matter fiber damage in left anterior thalamic radiation,left upper longitudinal tract and left uncinate tract in first-episod.e depression is mainly related to nerve circuits related to emotional response,and no correlation between cognitive function damage and white matter microstructure.
