The Study Of The Correlation Between Prognosis Of Sepsis And The TCM Crisis

Objective:To observe the distribution of the TCM crisis in patients with sepsis admitted to the emergency department of Guangdong Provincial Hospital of Traditional Chinese Medicine and ICU,and to analyze the correlation between the TCM crisis,APACHE-? score and SOFA score and patient outcome,and to explore the prognosis of sepsis.The correlation of the TCM crisis is expected to predict the condition and prognosis without APACHE-? score and SOFA score,and provide reference for clinical diagnosis and treatment.Methods:From February 2018 to January 2019,in the emergency department of the Guangdong Provincial Hospital of Traditional Chinese Medicine and the patients with sepsis admitted to the ICU,a total of 60 patients who met the inclusion criteria were screened for a prospective investigative study.First,according to the time of inclusion,from three time nodes(inclusion:Day 1,Day 3/Death,Day 7/Death),the incidence of related syndromes was assessed through self-made clinical observations,and the patient' s condition was also assessed.Perform APACHE-? score and SOFA score.After summarizing the relevant syndromes,the distribution of the dangers in the three assessments is obtained.The frequency analysis method was used to collect the distribution of common TCM crisis.Then,the relationship between the treatment outcome and the main indicators(SOFA score level,APACHE-?score level and risk)was analyzed.The analysis was performed by SPSS17.0 software.The statistical methods mainly included descriptive analysis,t-test,Mann-Whitney U test,binary logistic regression analysis,etc.P<0.05 was significant difference.Result:1.In the risk of sepsis patients,the top five were:116 cases(35.4%),63 cases(19.2%)with cold hands and feet,37 cases(11.3%)with limb edema,and 29 cases of asthma.(8.8%),21 cases(6.4%)of the disease,including consciousness,pulse,breath,limb circulation and limb condition.The risk of death in patients with death is higher than that of discharged patients.2.Comprehensive 3 evaluations:In the SOFA score,the frequency of occurrence of patient risk at SOFA score>6.5 is relatively higher than the frequency of occurrence of patient risk at SOFA score ?6.5.In the APACHE-? score,with the increase of the score,the frequency of limb edema and fever gradually decreased;the frequency of dysfunction and polymorphism gradually increased,the absence of the gods,the coldness of the hands and feet,the coldness of the limbs,and the large flesh.The frequency of occurrence of bone loss first decreases and then increases.3.In the three assessments,the difference of SOFA scores between the death group and the survival group was statistically significant(P<0.05).The average SOFA score of the former was greater than the latter,and the SOFA score of the death group was considered to be high.The SOFA score in the survival group;the Mann-Whitney U test showed a statistically significant difference in the APACHE-? score between the death group and the survival group(P<0.05),and the median value of the former was greater than the latter,and the death group was considered The APACHE-? score was higher than the APACHE-? score in the survival group.4.In the three assessments,both loss of consciousness and coldness of the hands and feet were risk factors for the treatment outcome(P<0.05).As the probability of occurrence of the two risks increased,the prognosis mortality of sepsis patients increased.Conclusion:1.Common TCM risk in patients with sepsis:loss of consciousness,coldness of hands and feet,edema of limbs,asthma,disease,involving patients' consciousness,breath,pulse,limb circulation and limb performance.2.The prognosis of sepsis has a certain correlation with the risk of TCM.Loss of weight and coldness of the hands and feet are the risk factors for the prognosis of patients with sepsis,and have a reference value for predicting the patient's condition and prognosis.
