Study On The Effect Of Three Health Education Methods On Patients With Type 2 Diabetes

Objective:To investigate the status of empowerment,self-management behavior,and knowledge of diabetes in patients with type 2 diabetes,and to analyze the influencing factors of empowerment;Compare the effects of three health education methods on patients with type 2 diabetes.Methods:In this study,quasi-experimental design was adopted.According to the inclusion and exclusion criteria,patients with type 2 diabetes who were admitted to the hospital from May 2018 to October 2018 were divided into groups according to the time of admission.Patients in the May?June group received traditional health education;patients in the July?August group received group education based on conversation map;patients in the September ? October group received one-to-one health education based on empowerment theory.Baseline survey was conducted before all intervention.The total intervening time was about 6 hours.The patients were followed up for 3 months after the intervention.The diabetes knowledge questionnaire,The Summary of Diabetes Self-Care Activities,Diabetes Empowerment Scale and related clinical examination results were collected at the follow-up.Excel and SPSS22.0 statistical software were used to collate and analyze the data,and t test,chi-square test,variance analysis,correlation analysis,multiple linear regression analysis and repeated measurement analysis of variance were used to analyze the results.Results:1.According to inclusion and exclusion criteria,145 patients with type 2 diabetes mellitus were investigated at baseline in this study.After 3 months of follow-up,15 people were lost.43 people were eventually completed in the routine health education group,44 in the Conversation Map group and 43 in the empowerment Education group.2.Patient baseline,the average score of the patient's empowerment scale was 3.40±0.34;The total score of self-management behavior was 20.15±8.19 points,of which the overall diet score was 1.69±2.41 points,the specific diet score was 6.59±3.24 points,the exercise score was 5.46±3.03 points,the blood glucose monitoring score was 1.16±2.06 points,and the foot care score was 5.26 ± 4.24 points;The score of diabetes knowledge questionnaire was 14.45 + 3.77 points.3.Correlation analysis shows that the patient's empowerment ability is positively related to the patient's self-management behavior and diabetes knowledge;multiple regression analysis of the factors influencing the empowerment ability shows that the patient's cultural level and self-management behavior are the influencing factors of the empowerment ability,and explain the total variation together31.3%.4.The baseline general data,disease-related data,blood glucose level,self-management behavior score,empowerment ability score and diabetes knowledge questionnaire score of the patients in the three health education intervention studies showed no statistical significance(P>0.05)and were comparable.5.There was no difference in the scores of diabetes knowledge between the three groups before and after the intervention of the three health education methods(P>0.05).The FPG was different between the groups at the time of January(P<0.05),and the P2 BG was different between the groups at the time of March(P<0.05);self-management behavior scores and empowerment ability after intervention were significantly different between groups(P<0.05).There were significant differences in FPG,P2 BG,self-management behavior scores,empowerment ability scores,and diabetes knowledge scores before and after intervention(P<0.05).6.After the intervention of the three health education methods,the indicators of the patients were better than the baseline level.There was no difference between theconversation map group and the empowerment education group(P>0.05),but they were better than the routine health education group.7.After the intervention of the three groups of patients,the trend of FPG and P2 BG decreased first and then increased,which was the lowest in January and still lower than the baseline in March.The trend of self-management behavior score,empowerment ability score and diabetes knowledge score increased first and then decreased.Highest in January and above baseline in March.Conclusion:1.The empowerment ability of type 2 diabetes belongs to the middle level,and the self-management behavior and knowledge of diabetes are poor.2.The empowerment ability of patients with type 2 diabetes is related to the patient's self-management behavior and knowledge of diabetes.3.The main influencing factors of the empowerment level of type 2 diabetes patients are: the patient's education level and self-management behavior score.4.Three health education methods can improve the blood sugar level of patients with type 2 diabetes,improve their self-management behavior,empowerment ability and knowledge of diabetes,and the effect of conversation map education and empowerment education is better than that of regular health education.There is no difference between the conversation map education and the empowerment education.
