A Study Of Ipad Video-education Combined With Health Diary On The Health Management Of Patients With Type 2 Diabetes Mellitus

ObjectiveTo explore the effects of health education via iPad video-education and health diary on type 2 diabetes mellitus patients about disease knowledge,self-efficacy,self-management ability and blood glucose control.To analyze the influence factors of health management effects and provide theoretical basis for extension nursing services in the later clinical study.MethodsA total of 116 type 2 diabetes patients were recruited from March to September 2018in the department of endocrinology at a Third-Grade Class-A hospital in Chengdu.Following principles of the randomized controlled trial,doctors of the outpatient department randomly divided patients into two groups and took them respectively into Ward 1?control group,n1=56 cases?and Ward 2?observation group,n2=60 cases?endocrinology department.Ordinary medical care and iPad-based health education in patient rooms were provided for both groups,while patients in the observation group needed to additionally finish health diaries with the assistance of nurses.On the first and the14th day of their hospitalization,patients were assessed with Diabetes Knowledge Test?DKT?,Diabetes Management Self-Efficacy Scale?DMSES?and Summary of Diabetes Self-Care Activities?SDSCA?.On the first,the 7th and 14thh day of their hospitalization,patients'fasting blood glucose?FBG?and 2-hours postprandial glucose?2hPG?were collected by nurses.Results1.Diabetes KnowledgeOn the 14th day of their hospitalization,the diabetes knowledge in both groups patients has improved?P<0.05?,and the observation group's knowledge was higher than the control group's?P<0.05?.2.Diabetes Management Self-Efficacy On the 14th day of their hospitalization,self-management efficacy?diet control,regular exercise,self-monitoring blood glucose,foot care,blood glucose treatment and medication adherence?in both groups patients has improved?P<0.05?.There was no statistical significance in medication adherence between the two groups,and the other self-management efficacy dimensions in the observation group patients were higher than those in the control group patients?P<0.05?.3.Diabetes Self-management AbilityThe control group patients'self-management capability in reasonable diet,medication adherence,and blood glucose monitoring has improved on the 14th day of their hospitalization,but there were no significant differences in regular exercise or foot care?P>0.05?.And the observation group patients'self-management capability has significantly improved?P<0.05?.There was no statistical significance in medication adherence?P>0.05?between the two groups,and self-management capability?reasonable diet,regular exercise,blood glucose monitoring and foot care?in the observation group patients were better than those in the control group patients?P<0.05?.4.Blood Glucose ControlThere were no significant differences in FBG or 2hPG between the two groups patients?P>0.05?on the first day and the 7th day of their hospitalization.The FBG and 2hPG of the observation group patients were lower than those in the control group?P<0.05?on the 14th day of their hospitalization.5.Analysis of Influencing FactorsMultiple linear regression was used to analyze the basic data and health management effect of patients in the observation group.The results showed that educational levels and number of complications were the factors that influenced patients'awareness of disease knowledge?P<0.05?.The health education experience and smoking history were the factors that affected self-management efficacy of the patients?P<0.05?.The number of complications,diabetes health education experience and income levels could affect patients'self-management capability?P<0.05?.ConclusionsThe form of health education combining iPad video with health diary not only provides a learning platform for patients to learn health knowledge,but also enables patients to participate in their own health management through the health diary with evidence-based and targeted measures.It can fully mobilize the patients'enthusiasm and self-confidence,help patients to form a healthy lifestyle and develop good behavior habits.It can effectively improve patients'diabetes knowledge,self-management efficacy and self-management capability.In addition,it also can help patients to control fasting blood glucose and 2-hours postprandial glucose.Factors such as patients'education level,income level,number of complications,smoking history,and health education experience can affect their self-health management.In the future clinical and extended nursing service work,we should provide an individualized health management program according to the above factors to improve patients'health management effect.
