Clinical Observation Of Tong Xie Yao Fang In The Prevention Of Tardive Diarrhea Toxicity Of Irinotean

ObjectiveTo study the effect of Tongxie Yaofang on delayed diarrhea caused by irinotecan,and further to explore the possible mechanism of delayed diarrhea caused by irinotecan and the feasibility of Tongxie Yaofang in clinical application.MethodsProspective randomized controlled trials were used to select patients with colorectal adenocarcinoma who met the criteria and diagnostic criteria for the study.The patients were divided into Chinese medicine treatment group and control group according to the hospitalization order and random number table method.The incidence of delayed diarrhea,the classification of delayed diarrhea,the changes before and after treatment of β-G activity,the myelosuppression(mainly white blood cells),and the physical state Karsness score(KPS)were used as indicators to measure the efficacy.Analyze the side effects of diarrhea in adjuvant chemotherapy.ResultsA total of 80 eligible cases were collected,including 40 cases in the treatment group and 40 cases in the control group.One chemotherapy cycle was observed.(1)Delayed diarrhea occurred in the two groups: After chemotherapy,there were 6 cases of delayed diarrhea in the treatment group,with an incidence of 15.00%,and 14 cases of delayed diarrhea in the control group,with an incidence of 35.00%.The total incidence of delayed diarrhea in the treatment group was significantly lower than that in the control group P=0.031(P < 0.05),with statistically significant differences.(2)The β-G activity in the two groups before and after treatment: Before treatment,compared with the control group,the activity level of β-G activity in the treatment group had no statistical difference,P=0.116(P > 0.05).After treatment,the activity level of β-G in the treatment group was lower than that in the control group,and the difference was statistically significant P=0.021(P < 0.05).(3)Two groups of myelosuppression condition in the two groups: After treatment,the totalincidence of myelosuppression in the treatment group has no difference between the control group,but the incidence of Ⅲ°+ Ⅳ°myelosuppression significantly lower than control group,P = 0.016(P < 0.05).(4)Leukocyte inhibition occurred in the two groups: 5 cases of leukocyte reduction in the treatment group after treatment,with a total incidence of 12.50%.13 cases of leukopenia in the control group after treatment,with a total incidence of 32.50%,and the total incidence of leukopenia in the two groups was statistically different,P=0.030(P<0.05).(5)KPS scores in the two groups before and after treatment: Compared with the control group,there was no significant difference in the KPS improvement data of the treatment group after treatment,but the KPS reduction index data of the treatment group after treatment was less than that of the control group P=0.032(P < 0.05),showing statistical significance.ConclusionTongxie Yaofang can prevent delayed diarrhea caused by irinotecan.Tongxie Yaofang can reduce the incidence of delayed diarrhea,inhibit β-G activity,reduce the incidence of leukopenia,reduce the incidence of Ⅲ ° + Ⅳ ° myelosuppression.And,it has a certain effect on improving patients’ KPS score,can improve patients’ quality of life,has the feasibility of clinical application and is suitable for clinical promotion.
