Objective: To explore the efficacy of triamcinolone acetonide combined with botulinum toxin A in the treatment of scars.Methods: Twenty New Zealand white rabbits.The experimental rabbits were divided into four groups: blank control group(injecting normal saline equivalent to botulinum toxin A and triamcinolone acetonide at scar site),botulinum toxin A group(injecting dose at scar site according to the principle of 3U/place),triamcinolone acetonide group(injecting dose at scar site according to the principle of 100ul/place),combined drug group(injecting at the same scar site the two drugs.Botulinum toxin A and triamcinolone acetonide were injected at the same dosage as those of single botulinum toxin A and triamcinolone acetonide.).The hypertrophic scar model of rabbit ear was established by referring to the literature.Eight scar wounds were created in each rabbit.Drugs or saline were injected into the rabbit ear on a fixed date.To observe and collect the physical evaluation indexes of scar: the color,texture and thickness of scar;to make and detect the biochemical indexes of scar: HE staining method was used to measure the number of fibroblasts,immunohistochemical SP method was used to detect the proportion of collagen I and III and the expression levels of vascular endothelial growth factor and TGF-beta 1Results:1.The results of scar thickness showed that the thickness of combined drug group was the lowest,which was lower than that of botulinum toxin group,triamcinolone acetonide group and blank control group,and the results were statistically significant.There was no significant difference in the thickness between botulinum toxin group and triamcinolone acetonide group.Compared with blank control group,botulinum toxin group and triamcinolone acetonide group had lower thickness,with statistical difference.2.The results of scar color and texture showed that the proportion of normal and pink was 62.50% and 65.79% respectively in combination group and triamcinolone acetonide group,52.50% and 33.33% in botulinum toxin group and control group,and 52.50%,55.00%,41.03% and 28.95% in combination group,triamcinolone acetonide group and blank control group,respectively.Obviously higher.3.The results of fibroblast count showed that the density of the combined drug group was the lowest,and the results were statistically significant.There was no significant difference between botulinum toxin group and triamcinolone acetonide group.The density of botulinum toxin group and triamcinolone acetonide group was lower than that of blank control group,and the results were statistically different.4.The results of type I and III collagen content and proportion showed that the combined use of type I and III collagen content group was higher than triamcinolone acetonide group,botulinum toxin group and blank control group,and the results were statistically significant.Type I and III collagen ratio: The combination group < triamcinolone acetonide group < botulinum toxin group < blank control group.The analysis of variance between groups showed that the combination group,triamcinolone acetonide group,botulinum toxin group and blank control group had statistical differences.However,there was no significant difference between the combined drug group and triamcinolone acetonide group and botulinum toxin group.5.The results of TGF-beta 1 showed that the concentration of combined drug group was the lowest,which was lower than that of botulinum toxin group,triamcinolone acetonide group and blank control group,and the results were statistically significant.There was no significant difference between botulinum toxin group and triamcinolone acetonide group.The results of botulinum toxin group and triamcinolone acetonide group were significantly different from those of the blank control group.6.The results of vascular endothelial growth factor showed that there was no significant difference among the combined drug group,triamcinolone acetonide group and botulinum toxin group.Conclusions: By establishing an animal model of rabbit ear scar and using the physical and biochemical indexes of scar evaluation,it is proved that botulinum toxin A combined with triamcinolone acetonide is more effective in preventing the occurrence of hypertrophic scar in rabbit ear than single drug.The process and conclusion may further inspire clinical research.
