Objective:To investigate the expression of mitochondrial transcription factor A?TFAM?in peripheral blood of patients with chronic heart failure?CHF?.Methods:Sixty patients with CHF were admitted to the Department of Cardiology,Affiliated Hospital of Chengde Medical College,and selected as the CHF group,while eighty-two patients with normal heart function were selected as the control group.All patients signed informed consent,and this study had been approved by the Hospital Ethics Committee.The data of participants,which included baseline characteristics,blood indexes and echocardiographic indicators,were recorded.Meanwhile,peripheral blood samples were collected.The specific amplification and dissolution curves of TFAM in peripheral blood of the two groups were detected by RT-PCR.The CT values of each sample were obtained.The CT value represents the number of PCR cycles when the specimen expands to the specified threshold value.The relative content of TFAM was calculated by formula,2-??CT.Then we analyzed the content changes of TFAMmRNA in peripheral blood of patients with or without CHF.Results:1.General clinical characteristics:Compared with the control group,the CHF group had lower BMI?P<0.05?;The patients with type 2 diabetes mellitus were significantly higher in CHF group than those in control group?P<0.05?.2.The characteristics of laboratory indicators and TFAMmRNA content in peripheral blood:Compared with the control group,the CHF group had higher neutrophils,creatinine,uric acid,glutathione transaminase,brain natri-uretic peptide?BNP??P<0.05,respectively?.The CHF group,compared with the control group,had lower lymphocytes,total cholesterol,triglyceride,low density lipoprotein cholesterol,TFAMmRNA content?P<0.05,respectively?.3.The characteristics of echocardiographic indicators:Compared with the control group,the CHF group had lower ejection fraction?LVEF??P<0.05?and higher left ventricular end-diastolic diameter?LVEDD??P<0.05?.4.The content of TFAMmRNA and BNP levels in New York Heart Association?NYHA?grading groups:There were significant differences in the levels of BNP and TFAMmRNA content among NYHA grading groups?P<0.05?.The level of BNP showed the same change trend with the NYHA grade,while the content change of TFAMmRNA was different.5.The content of TFAMmRNA in different LVEF and LVEDD groups of CHF patients:The TFAMmRNA content in the patients with LVEF?40%was lower than that of the patients with LVEF>40%?P<0.05?.The content of TFAMmRNA in the patients with LVEDD 50-70 mm was lower than that of the patients with LVEDD?50 mm?P<0.05?;the content of TFAMmRNA in peripheral blood of the group with LVEDD?70 mm was lower than that of the group with LVEDD?50 mm and the group with LVEDD 50-70 mm?P<0.05?.6.The correlation between TFAMmRNA content and variables:The study showed that the content of TFAMmRNA was positively correlated with LVEF?r=0.462,P<0.05?,negatively correlated with LVEDD,BNP and diabetes mellitus?r=-0.314,-0.279,-0.177,P<0.05,respectively?.Conclusion:The expression of TFAM in peripheral blood of patients with CHF is lower than that in the control group.TFAMmRNA in peripheral blood plays an important role in the pathological changes of CHF and may become a new marker and therapeutic target of CHF.
