Objective:To explore the association between Calcium-Phosphorus Metabolism and serum microinflammatory index NLR in Maintenance Hemodialysis Patients.Method:A total of 84 patients undergoing MHD treatment were enrolled.General data and clinical indicators of calcium and phosphorus metabolism were collected.Serum NLR was calculated in MHD patients.C-reactive protein was detected by ELISA;Grouping according to whether calcium(Ca),phosphorus(P)and parathyroid hormone(PTH)meet the standard(Ca 2.1-2.5mmol/L;P 1.13-1.78mmol/L;PTH 130-600pg/ml).All the three indicators met the criteria were divided into standard group(28 cases)and more than or equal to one did not meet the criteria were divided into non-standard group(56 cases).To compare the difference of each index between the standard group and non-standard group,and to analyze the correlation of microinflammation index NLR with each index.Analyze the independent influencing factors of NLR.Results:1.Comparison of general data between standard group and non-standard groupThere were 28 MHD patients in the standard group,including 14 males and 14 females,with an average age of 63.07±11.05 years and an average HD time of 33.5(17.25,55)months;There were 56 MHD patients in the non-standard group,including 29 males and 27 females,with an average age of 58.34±12.47 years and an average HD time of 33.5(16.25,50.25)months.There was no significant difference in age,sex and HD time between the standard group and the non-standard group(P>0.05).2.Comparisons of clinical indicators between standard group and non-standard groupThere was no significant difference in Hb,Alb,UA,Alp,FERR,KT/V between the standard group and the non-standard group(P>0.05).The NLR value of standard group is lower than that of non-standard group[(3.48±1.17)vs(4.08±1.33),P<0.05];The CRP value of standard group is lower than that of non-standard group[(2.03±0.39)vs(2.28±0.43),P<0.05];The calcium value of standard group is higher than that of non-standard group[(2.27±0.08)vs(2.10±0.26),P<0.05];The phosphorus value of standard group is lower than that of non-standard group[1.57(1.37,1.67)vs2.37(2.09,2.83),P<0.05];The Ca×P value of standard group is lower than that of non-standard group[(42.88±5.53)vs(64.82±16.92),P<0.05];The PTH value of standard group is lower than that of non-standard group[258.75(173.37,375.85)vs385.50(262.33,639.32),P<0.05];The Scr value of standard group is lower than that of non-standard group[(778.48±180.14)vs(921.62±219.50),P<0.05].3.The correlation between serum NLR level and other indicators in patients undergoing MHDSerum NLR level was positively correlated with CRP(r = 0.632,P<0.001),P(r = 0.338,P<0.01),Ca×P(r = 0.313,P=0.004);NLR was negatively correlated with Alb(r =-0.392,P<0.01);NLR was not correlated with serum Ca(r =-0.115,P = 0.296),PTH(r = 0.072,P = 0.514),Alp(r =0.098,P=0.375).Using NLR as dependent variable,multiple stepwise regression analysis showed that serum CRP?P and Alb were independent influencing factors of NLR(F=23.758,P<0.001;t= 5.738,P<0.001;t=2.369,P=0.020;t=-2.169,P=0.033).Conclusion:1.Serum phosphorus,calcium-phosphorus product,albumin were associated with serum NLR in MHD patients.The NLR level of patients reaching the reference range was low.2.High level of NLR in MHD patients is associated with higher serum phosphorus,calcium-phosphorus product and lower albumin.3.Within a certain range,the higher the serum CRP,the higher the serum NLR level in MHD patients.4.Serum phosphorus,albumin and CRP independently influence NLR level in MHD patients.
