Objective To investigate the pathogens and risk factors of patients with severe acute pancreatitis(SAP)to provide guidance for clinical treatment.Methods A retrospective analysis was conducted on a total of 438patients with SAP in our hospital from Feb.2011 to Jan.2018.The samples including ascites,sputum,and blood were collected.Distribution of pathogens,drug resistance,and factors related to SAP secondary infection were analyzed.Results The total infection rate was 49.09%,including 44.29%bacterial infection rate and 22.60%fungal infection rate.A total of 625pathogens were cultured,with 333 strains of Gram-negative bacteria(G~-bacteria,53.28%),171 strains of Gram-negative bacteria(G~+bacteria,27.36%),and 121 strains of fungi(19.36%).G~-bacteria are highly resistant to?-lactams antibiotics,with its resistance rate to enzyme inhibitors slightly lower.G~+bacteria have low resistance rates to vancomycin,linezolid,and tigecycline.The univariate analysis and multivariate logistic regression analysis showed that the independent risk factors of patients with SAP secondary infection include MODS,APACHE II score?11,fasting time?3 weeks,mechanical ventilation and surgery(P<0.05).By stratified analysis of infection in different parts,it was found that MODS,fasting time?3 weeks,mechanical ventilation and surgery was the independent risk factors of patients with SAP secondary infection(P<0.05).Conclusions SAP has a high infection rate and complex risk factors.It is necessary to prevent risk factors,improve pathogens and seek proper medication treatment as soon as possible.For patients with use of invasive operators,it is necessary to operate aseptically and prevent iatrogenic infection.
