Effects Of Large Volume Lung Lavage On Lung Function And Changes Of Related Factors

Objectives The purpose of this study was to investigate the effects of high-volume lung lavage on lung function and related factors,and to provide a basis for reducing the body’s inflammatory response and improving lung function.Methods The patients with occupational pneumoconiosis were selected from March 2018to December 2018 in Emergency General Hospital for the study,and they were given large volume whole lung lavage.The pulmonary function was measured before,one month after and three months after lavage,including maximal ventilatory volume（MVV）,forced expiratory vital capacity（FVC）,forced expiratory volume in 1 second（FEV₁）,forced expiratory volume in 3 second（FEV₃）,forced expiratory volume in 1 second（FEV₁/FVC）,forced expiratory volume in 3 second（FEV₃/FVC）,maximal mid-expiratory flow（MMEF）,peak expiratory flow（PEF）,maximal expiratory flow in 75%vital capacity（MEF75）,maximal expiratory flow in 50%vital capacity（MEF50）,maximal expiratory flow in 25%vital capacity（MEF25）,residual volume/total lung capacity（RV/TLC）,functional residual capacity（FRC）,alveolar ventilation（VA）and carbon monoxide diffusing capacity（DLco）.Venous blood was collected before,one month after and three months after lavage.The supernatant of venous blood was centrifuged and the contents of IL-1beta,TNF-alpha,IL-10,ROS,MDA and SOD in serum were measured by ELISA.Results 1 General information shows that the number of participants in the study was 46,height（167.61±6.31）cm;body weight（70.41±8.19）kg;age（47.83±6.02）years;dusting age（14.93±7.64）years.Among them,37 patients with coal worker’s pneumoconiosis and 9 patients with silicosis,including 16 patients in the first phase,26patients in the second phase,and 4 patients in the third phase.2 There were significant differences in VA%and RV/TLC%between lung function indexes before and after lavage（P<0.05）.The VA%values at three months after lavage were higher than those before lavage（P<0.05）,and RV/TLC%values.three months after lavage,lower than before lavage（P<0.05）,that is,lung lavage can increase effective ventilation in the lungs,reduce the ratio of residual air volume to total lung volume,damage degree to lung function and ventilation function index The abnormal rate was not statistically significant（P>0.05）.It can be seen from the numerical changes that lung lavage has an improvement trend on lung function damage and ventilatory funxction.3 There were significant differences in serum IL-1β,TNF-α,MDA and SOD groups before and after lavage（P<0.05）;IL-1βand lavage one month after lavage and three months after lavage Before the comparison,the serum content decreased（P<0.05）;the TNF-αvalue at three months after lavage was lower than before lavage（P<0.05）;the MDA at three months after lavage was lower than one month after lavage（P<0.05）,the SOD value at three months after lavage was higher than that before lavage and one month after lavage（P<0.05）.The abnormal rate of IL-1βand TNF-αbefore and after lavage was statistically significant（P<0.05）,consistent with differences in IL-1βand TNF-αvalues before and after lavage.4 To explore the relationship between the changes of lung function and serum cytokines before and after lavage,the VA%value before lavage was negatively correlated with IL-1β（r=-0.343,P<0.05）,and the VA%value was negatively correlated with TNF-α.（r=-0.366,P<0.05）,RV/TLC%was positively correlated with MDA（r=0.440,P<0.05）;VA%was negatively correlated with IL-1βone month after lavage（r=-0.345,P<0.05）;VA%and RV/TLC%at three months after lavage and serum cytokines showed no correlation between lung function index and each factor（P>0.05）,TNF-αafter lavage The IL-1βand MDA values decreased,the VA%value increased,and the RV/TLC%value decreased.Conclusions 1 Large-capacity whole lung lavage can improve the lung function index of patients with pneumoconiosis.2 After lung lavage,the related inflammatory factors in patients have a decreasing trend,suggesting that this technology can reduce the body’s inflammatory level and improve lung function.Figure 3;Table 14;Reference 160...
