Objectives To make a comparative study of the relationship between the ADP-induced platelet aggregation rate and recent major adverse cardiovascular events in acute coronary syndrome patients with dual antiplatelet therapy(aspirin,ticagrelor or clopidogrel).Methods This study included patients with acute coronary syndrome who were hospitalized in Hebei Provincial People's Hospital from September 2016 to September 2017 and tested for platelet aggregation.According to the result of ADP-induced aggregation rate,patients were successively divided into group A(ADP aggregation rate?46%)who reached the standard and group B(ADP aggregation rate>46%)who didn't reach the standard.In the end,there were 160 cases in group A and 172 cases in group B.The incidence of MACE(angina,recurrent myocardial infarction,stent thrombosis,stroke,death)was observed between groupA and group B.Results 1 The clinical incidence of MACE within six months of group A was sign ificantly less than group B(28.1%vs.41.3%,P=0.012).The clinical incidence of a ngina within six months of group A was significantly less than group B(24.4%vs.35.5%,P=0.028).There was no significant difference in the incidence of death,recur rent myocardial infarction,stent thrombosis and stroke between the two groups(P>0.05).2 In order to screen the risk factors for predicting recent clinical end-point events,the variables of univariate analysis were included in the multivariate COX proportional hazard regression analysis.The variables included group,history of perc utaneous transluminal coronary intervention,cardiac shock,hypertension,Serum crea tinine,left atrial diameter.After multivariate COX regression analysis was performed,group[HR=1.754,95%C/(1.193-2.578),P=0.004],hypertension[HR=2.373,95%CI(1.445-3.897),P=0.001]and Serum creatinine([HR=1.011,95%CI(1.004-1.019),P=0.004])were independent risk factors for predicting MACE events.There were fewer cardiovascu lar adverse events in group A than in group B.Conclusions 1 Platelet function was reflected by detecting plateletaggregation rate,which helped to find patients with high platelet reactivity(HPR).2 The anti-platelet therapy was more effective in acute coronary syndrome patients with ADP platelet aggregation rate than that in acute coronary syndrome patients,and the incidence of MACE events might be smaller.3 ADP-induced platelet aggregation rate didn't re ach the standard,hypertension history and serum creatinine were independent risk f actors for predicting the recent occurrence of MACE.Figure1;Table5;Reference 140...
