The Role Of Heparin-binding Protein In Children With Sepsis

Objective to investigate the relationship between the level of heparin binding protein(HBP)in children with sepsis and severity,prognosis of sepsis.Methods One hundred and five children admitted in the pediatric intensive care unit were divided into three groups:critical children without sepsis group(Group A),sepsis group(Group B),severe sepsis group(including severe sepsis with shock)(Group C).30 healthy children served as control.The level of HBP in Group C were detected by the enzyme-linked immunosorbent assays in the first day(d1),third day(d3)and fifth day(d5).HBP in other groups were detected by the same way in the first day.Then collecting in 24 h the pediatric critical illness score(PCIS),the pediatric risk of mortality scores(PRISM ?),C-reactive protein(CRP),procalcitonin(PCT).Results The level of HBP in Group B and C are higher than Group A and D(P<0.05).There is no significant differences between Group B and C(P=0.21).There is no significant differences in HBP concentrations at d1 between 28-day non-survivors and survivors(P=0.54).But HBP concentrations at d5 were significantly higher among 28-day non-survivors as compared to survivors(P<0.05).Relation analysis was studied between HBP and PRISM ?,PCIS by Spearman rank correlation coefficient.There is no correlation.We also studied the early prognostic value on sepsis by establishing receiver operating characteristic curve(ROC).The prognostic value of HBP is better than CRP and PCT in diagnosis of sepsis in children(AUC=0.82).Conclusions The level of HBP in sepsis and severe sepsis patients are higher than critical patients without sepsis and health group.HBP concentrations has correlation with prognosis of sepsis,no correlation with sepsis severity.The prognostic value of HBP is better than CRP and PCT in diagnosis of sepsis.
