Objective: To observe the clinical effect of resveratrol(RES)combined with Sertraline hydrochloride in the treatment of post-stroke depression(PSD),Detect the level of norepinephrine(NA),5-serotonin(5-HT),brain-derived neurotrophic factor(BDNF)in patients with PSD.Investigate the clinical efficacy and correlation with NA,5-HT,BDNF of resveratrol combined with sertraline hydrochloride in the treatment of PSD.Methods: 1.The source of the study was 88 patients with PSD admitted to Affiliated Hospital of Youjiang Medical University for Nationalities for the period from August 2017 to November 2018.According to the order of admission,the patients are divided into 22 cases in the control group,22 in the resveratrol group,21 in the sertraline hydrochloride group and 23 in the combined group by the “distribution principle with the smallest imbalance index”.The patients in the control group were treated with routine neurological rehabilitation exercises and drug.On the basis of conventional treatment in the control group,the resveratrol group was treated with resveratrol capsules,the sertraline hydrochloride group was orally administered with sertraline hydrochloride tablets,and the combined group was treated with oral resveratrol capsules and sertraline hydrochloride tablets.All four groups were treated continuously for 8 weeks.2.The antidepressant effect was evaluated by the Hamilton Depression Scale(HAMD-24)commonly used at domestic and foreign.The neurological rehabilitation was evaluated by the National Institute of Health's Stroke Scale(NIHSS).The ability of daily living was assessed using the Simple Daily Living Ability Scale(ADL).The patient's sideeffects during the study were recorded using the Antidepressant Side Effects Scale(SERS).3.Serum-derived neurotrophic factor(BDNF),serotonin(5-HT),norepinephrine(Noradrenaline)were detected by enzyme-linked immunosorbent assay(ELISA)at different time points.,NA)expression level.Results:1.the HAMD-24 items score(1)Comparison in the same group: At the end of the fourth and eighth week,the HAMD scores of the resveratrol group,sertraline hydrochloride group and the combined group were significantly lower than those before treatment and at the fourth weekends,and the difference was statistically significant(P<0.05).(2)Comparison among groups: At the end of the 4th week and 8th week,the difference between the sertraline hydrochloride group and the control group was statistically significant(P<0.05),there was a statistically significant difference in the comparison of the combined group with the control group and the resveratrol group(P<0.05).At the end of the 8th week,the difference between the resveratrol group and the control group was statistically significant(P<0.05),there was a statistically significant difference between the sertraline hydrochloride group and the resveratrol group(P<0.05).2.the NIHSS score(1)Comparison in the same group: At the end of the fourth and eighth week,the NIHSS scores of the study group were significantly lower than those before treatment and at the end of the 4th week,and the difference was statistically significant(P<0.05).(2)Comparison among groups: At the end of the fourth and eighth weekends,there was significant difference between the sertraline hydrochloride group and the control group(P<0.05),the combined group was compared with the control group and the resveratrol group respectively,the difference was statistically significant(P<0.05).At the end of the 4th week,the difference between the sertraline hydrochloride group and the resveratrol group wasstatistically significant(P<0.05).3.the ADL score(1)Comparison in the same group: At the end of the fourth and eighth weekends,the ADL scores of the study group were significantly higher than those before treatment and at the end of the 4th week,and the difference was statistically significant(P<0.05).(2)Comparison among groups: At the end of the fourth and eighth week,there was a statistically significant difference between the sertraline hydrochloride group and the control group(P<0.05),the combined group was compared with the control group and the resveratrol group respectively,the difference was statistically significant(P<0.05).4.the serum NA detection(1)Comparison in the same group: At the end of the fourth and eighth week,the serum NA levels in the control group and the study group were significantly higher than those before treatment and at the end of the 4th week,and the difference was statistically significant(P<0.05).(2)Comparison among groups: At the end of the 4th and 8th week,the resveratrol group was compared with the control group and the sertraline hydrochloride group respectively,the difference was statistically significant(P<0.05),the combined group was compared with the control group,the resveratrol group and the sertraline hydrochloride group respectively,the difference was statistically significant(P<0.05).At the end of the 8th week,there was a statistically significant difference between the sertraline hydrochloride group and the resveratrol group(P<0.05).5.the serum 5-HT detection(1)Comparison in the same group: At the end of the fourth and eighth week,the serum5-HT levels in the control group and the study group were significantly higher than those before treatment and at the end of the 4th week,and the difference was statistically significant(P<0.05).(2)Comparison among groups: At the end of the 4th and 8th week,the sertraline hydrochloride group was compared with the control group and the resveratrol group respectively,the difference was statistically significant(P<0.05),the combined group was compared with the control group,the resveratrol group and the sertraline hydrochloride group respectively,the difference was statistically significant(P<0.05).6.the serum BDNF detection(1)Comparison in the same group: At the end of the fourth and eighth week,the serum BDNF levels in the control group and the study group were significantly higher than those before treatment and at the end of the 4th week,and the difference was statistically significant(P<0.05).(2)Comparison among groups: At the end of the 4th and 8th week,there was a statistically significant difference between the resveratrol group and the control group(P<0.05),there was a statistically significant difference between the sertraline hydrochloride group and the control group(P<0.05),the combined group was compared with the control group,the resveratrol group and the sertraline hydrochloride group respectively,the difference was statistically significant(P<0.05).7.The incidence of side reactions: the incidence of side reactions in the resveratrol group was 4.55%,the incidence of side reactions in the sertraline hydrochloride group was 19.05%,the incidence of side reactions in the combined group was 13.04%.There was no significant difference in side effects among the study group(P>0.05).8.the total effective rate:At the end of the 4th and 8th week,the total effective rate of the combined group was 82.61% and 91.30% respectively,the total effective rate of the sertraline hydrochloride group was 61.90% and 76.19%,respectively,the total effective rate of the resveratrol group was 31.82% and 54.54%,respectively,the total effective rate of the control group was 9.09% and 13.64%,respectively,there was significant difference between the sertraline hydrochloride group and the control group(P<0.0083),there was a statisticallysignificant difference in the comparison of the combined group with the control group and the resveratrol group(P<0.0083).At the end of the 8th week,the resveratrol group compared with the control group was statistically significant(P <0.0083).Conclusion:1.Resveratrol is effective in the treatment of PSD and may exert antidepressant effects by increasing the levels of NA,5-HT and BDNF.2.Resveratrol combined with sertraline hydrochloride has good antidepressant effect and tolerance in the treatment of PSD,and its clinical efficacy is better than single administration of resveratrol or sertraline hydrochloride.3.Anti-PSD treatment can promote neurological rehabilitation and improve daily living ability.
