| Objective: Efficacy and complications of CD19 chimeric antigen receptor T cells in the treatment of recurrent/refractory acute lymphoblastic leukemia.Methods:Retrospective analysis was performed on the efficacy and complications of 14 cases of refractory recurrent acute lymphoblastic leukemia treated with CD19 CAR-T cells in the hematology department of the second hospital of hebei medical university from January 2017 to January 2019.Results:1.Among the 14 patients,1 patient developed grade 5 CRS reaction in a short period of time,and 13 patients could be evaluated for efficacy for 28 days.The total response rate of 28 d was 9/14,in which 7 cases were complete remission,2 cases were partial remission,and 4 cases were invalid.2.All 14 patients(100%)had symptoms attributable to CRS: 2 had grade 1(14%),6 had grade 2(43%),3 had grade 3(21%),2 had grade 4(14%),and 1 had grade 5(8%).3.Fever occurred in all 14 patients.Nine patients(64%)developed different degrees of liver dysfunction.Conclusions:1.To determine the efficacy of CD19-CAR-T cells in the treatment of recurrent/refractory acute lymphocytic leukemia.2.CAR-T cells were maintained in vivo for up to 3 months.The recurrence rate was 21% at 3 months,and hematopoietic stem cell transplantation should be actively involved to achieve the goal of radical treatment.3.The main symptom of CRS is fever,and the most common organ function injury is liver function injury.4.IL-6,IFN-γand IL-10 were significantly increased in CRS.The highest levels of IL-6,IFN-γand IL-10 in the grade 3,4 and 5 CRS group were significantly higher than those in the level 1 and 2 CRS grade.CRS significantly increased CRP and ferritin. |
