OBJECTIVE:This study intends to use a multimode fundus image based on optical coherence tomography angiography(OCTA)to perform fundus examination of diabetic retinopathy(DR)patients with retinal neovascularization(Retinal Neovascularization).,RNV)is the main observation index,taking " Huayulishui Mixture " as an intervention means to observe the inhibitory effect of " Huayulishui Mixture " on retinal neovascularization,and at the same time,the evaluation of optical coherence tomography angiography(OCTA)in the classification,prognosis and efficacy judgment of diabetic retinopathy was evaluated.The application value of optical coherence tomography angiography in fundus diseases was evaluated objectively,and clinical diagnosis and follow-up were guided.METHODS: In the ophthalmology inpatients of the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine,patients with retinal neovascularization in patients with proliferative diabetic retinopathy(DR5 grade)were enrolled according to the inclusion and exclusion criteria.The Chinese medicine Huali hydrating agent combined with vitreous cavity injection was used.Intramuscular or intravitreal injection of ranibizumab as an intervention(two groups of intravitreal ranibizumab in a single dose of 0.5 mg / 0.05 ml),comparing the two treatments to compare the regressive effects of neovascularization and Protection of the patient's visual function.The primary outcome measures included best corrected visual acuity(BCVA),fundus photography,retinal neovascular morphology(vascular density),and macular central retinal thickness(edema)before and after intervention.RESULTS:(1)Best corrected visual acuity(BCVA): After treatment,the visual acuity of the subjects in the two groups was improved.Compared with the Log MAR values of the best corrected visual acuity(BCVA)at the five follow-up time points of 1W,2W,1M,2M,and 3M before treatment.There were statistical differences,and the visual acuity was more significant in the treatment group than in the control group.(2)Central retinal thickness(CRT)of the macula: After treatment,the macular edema of the two groups was reduced compared with that before treatment.There were significant differences in CRT values between the five follow-up time points before treatment and 1W,2W,1M,2M,and 3M after treatment..It can be said that the treatment group can significantly reduce macular edema compared with the control group.(3)Changes in retinal neovascular density in the observation area: After treatment,the retinal neovascularization density in the observation area of the two groups was lower than that before treatment.Five follow-up time before treatment and 1 W,2 W,1 M,2 M,and 3 M after treatment.There were statistical differences in the changes in point retinal neovascular density.The changes of neovascular density before treatment were compared between the two groups at the five follow-up points.The treatment group was significantly different from the control group,indicating that the treatment group could significantly reduce the neovascular density.(4)Safety evaluation: 30 patients with 30 eyes underwent a total of 30 "vitreous cavity injection(Raychemizumab)" injection therapy,1 case of subconjunctival hemorrhage,3 subjects complained of the front of the shadow,considered After liquefaction of the vitreous,the subjects did not find postoperative endophthalmitis such as redness,pain,sudden loss of vision,and no systemic adverse reactions related to the drug,indicating vitreous cavity injection and ranibizumab treatment.Diabetic retinopathy has a certain degree of safety.Conclusion: It is safe and effective to treat retinal neovascularization(RNV),diabetic macular edema(DME)and visual impairment caused by diabetic retinopathy with the Chinese medicine Huali hydrating agent combined with vitreous cavity injection(Rayrazumab),specifically to reduce Macular edema,inhibits neovascularization and improves visual function impairment in patients.
