| The Yi people,with a population of more than 8 million,are mainly distributed between Plateau and coastal hills in Sichuan,Yunnan,Guizhou and Guangxi provinces,and are one of the representative ethnic minorities in southwest China.The existing Yi Medicine Book of Ming Dynasty is the earliest ancient book of Yi medicine discovered at present.It records the experience of Yi people in treating diseases and using drugs before the 16 th century.Based on the ancient astronomical calendar,the October solar calendar became the characteristic medical theory of Yi medicine.At present,222 kinds of ancient books of Yi medicine have been collected(Yi and Chinese editions).At the same time,"traditional Yi medicine" has become the intangible cultural heritage of Sichuan Province,and "Yi medicine" is the intangible cultural heritage of Yunnan Province.With the discovery of appropriate technology of Yi medicine,the "water plaster therapy" of appropriate technology of Yi medicine in Yunnan has been selected as the national intangible cultural heritage protection project.Two special Yi medical hospitals have been established in China,namely,Yunnan Yi Medical Hospital(Grade III A)and Liangshan Yi Autonomous Prefecture Yi Medical Hospital(Grade III B).The establishment of clinical institutions of Yi medicine is constantly improving.Previously,based on the Yi medical literature and field investigation in the Yi inhabited areas,the exploration and collation of appropriate technologies such as Yi herbal moxibustion and Yi BIMOSUNI medicine have received important support from the State Administration of Traditional Chinese Medicine and other state-level funds.How to realize efficient knowledge discovery and scientific evaluation of Yi medical experience in a large number of medical literature books? Is it effective for the treatment of gastrointestinal dysfunction after abdominal surgery with appropriate techniques of herbal ironing? All of them are problems that need to be explored urgently.Objective:1.Based on the modern literature of Yi medicine,the empirical prescriptions of Yi medicine were screened,and the database was constructed and analyzed by using TCMISS platform.The medication regularity was studied,including statistics of high frequency drugs,drug combination analysis,statistics of main diseases and so on.2.On the basis of ancient books and modern literature of Yi medicine,we sorted out and screened the common Yi medicine,counted its basic attributes,and explored and collated the relationship between the basic attributes of Yi medicine,such as flavor,return,medicinal properties,main treatment,efficacy,etc.to obtain quantitative and visual research results.3.To observe the clinical efficacy of Yi herbal ironing and Chinese herbal fennel hot ironing in the treatment of gastrointestinal dysfunction after abdominal surgery,and to verify its clinical application value.Methods:1.Collecting the composition of prescriptions of Yi medicine recorded in the literature such as "Yi Medicine Prescription" and "Yi Medicine Prescription Collection",and entering TCMISS to construct the database of prescriptions of Yi medicine.Based on the rules and methods of software mutual information method,869 prescriptions of Yi medicine were sorted out and analyzed,and the frequency of commonly used drugs,drug combination forms,medication rules and statistics of main diseases were summarized.2.When choosing the Latin scientific names of Yi medicines contained in the Chinese National Pharmacopoeia as the basis,the original database is formed.The content attributes include the source of medicinal materials,Latin scientific names,medicinal parts,Erqi,Six Tastes,return routes,main diseases,etc.Then the original text information is decomposed and transformed into informational field data,which is beneficial to the computer.The identified database of Yi medicine contains 489 information of Yi medicine.Visual FoxPro 9.0,Gephi 0.9.2 and other software were used to count the attributes of Yi medicine,and to quantify and visualize the semantic relationship between taste,return,efficacy and dominance.3.Choose the commonly used Yi medicine,Potamogeton and Mujiangzi.After grinding the Potamogeton into Potamogeton velvet,add salt to heat conduction,mix them together and wrap them in non-woven bags.After heating at a certain temperature in the microwave oven,use the technique to rotate and move the patient's abdomen.According to the inclusion and exclusion criteria established in this study,125 patients with abdominal surgery were selected and randomly divided into treatment group(Yi medicine Huo Cao ironing)40 cases,control group(Chinese medicine fennel hot ironing)40 cases and blank group(without any treatment)45cases.All three groups were treated for the first time from 6 hours after operation until the patient developed anal exhaust as a sign of termination of treatment.The recovery time of anal exhaust,bowel sounds,defecation and total symptoms were compared among the three groups.Results:1.869 prescriptions of Yi medicine involve 905 kinds of medicines and food,of which 30 kinds are used more than or equal to 9 times,of which alcohol is the most commonly used.There are 20 combinations of commonly used drugs in Yi medicine,and most of the combinations of prescriptions in Yi medicine are drugs with the functions of activating blood circulation and removing blood stasis,promoting qi andrelieving pain,dispelling wind and activating collaterals.The main diseases treated by Yi doctors are gynecological diseases,osteoarthritis and ophthalmopathy.In terms of the number of prescriptions,the main prescriptions of Yi medicine are small prescriptions under three kinds of medicines.Ninety-five alcoholic prescriptions of Yi medicine were analyzed.They were mainly used for gynecological diseases,osteoarthropathy,injury of falls and so on.The compatibility of alcoholic drugs was mostly combined with drugs of promoting blood circulation and promoting Qi circulation,clearing heat and warming the middle temperature.From the gynecological disease medication is the use of food,more compatible with drugs with the effect of promoting blood circulation and removing blood stasis,tonifying qi and blood,clearing away heat and detoxification.2.A total of 24 ancient and modern Yi medicine literature books were collected and sorted out,and a total of 489 kinds of Yi medicine database were constructed.Through analysis,it was concluded that the Yi medicine was mainly plant medicine,of which 26.94% and 26.79% were herbal and root medicinal parts;the Yi medicine was cold and hot,and cold medicine was more than hot medicine.The six flavors of Yi medicine are mostly bitter.The most common Yi drugs are Guiyi Road,followed by Guixin Road,Small Intestine Road and Gastric Road.There are 70 kinds of Yi drugs recorded for toxicity.Ganlu in Yi medicine is closely related to other medicinal flavors,especially bitterness and sweetness.Yi medicine is most commonly used in the treatment of fall injury.Its treatment may be related to the efficacy of Yi medicine in dispelling wind and dampness,clearing fire and detoxifying,promoting blood circulation and relieving pain.3.Analyzing the clinical effect of this study,we can find that the total effective rates of the treatment group and the control group were 100% and 96.7% respectively within 72 hours after the treatment of hay ironing and fennel hot ironing.(1)Comparison of recovery time of anal exhaust: There was significant difference in recovery time of anal exhaust among the three groups(F=9.730,P<0.05).LSD of post-test for recovery time of anal exhaust in the three groups showed that recovery time of anal exhaust in blank group was longer than that in control group and treatment group(P<0.05).There was no significant difference between control group and treatment group.(P>0.05)(2)Postoperative defecation score comparison: There was significant difference in defecation recovery among the three groups(F=5.073,P<0.05).By comparing the defecation recovery score of the three groups with LSD of post-test,we can see that the defecation recovery score of the blank group was higher than that of the control group and the treatment group(P<0.05).There was no significant difference between the control group and the treatment group(P>0.05).(3)Comparison of bowel sounds recovery score after operation: There was significant difference in the recovery score of anal and intestinal sounds among the three groups(F=14.590,P <0.05).The LSD comparison of the three groups showed that the recovery score of bowel sounds in the blank group was higher than that in the control group and the treatment group(P<0.05).The recovery score of bowel sounds in the control group was higher than that in the treatment group,and the difference was significant.There was statistical significance(P <0.05).Conclusion:1.Through data mining technology,the prescription of Yi medicine is mainly based on commonly used medicinal materials,local plants,food and seasoning in daily life.The main treatments are gynecological diseases,osteoarthritis and other common diseases.The results are similar to those recorded in ancient books of Yi medicine and clinical practice of Yi medicine.2.The main medicinal properties of Yi drugs are cold and heat,bitter and sweet taste,pancreas,heart and small intestine are the main routes to return.The toxicity of Yi drugs is mainly toxic,minor and severe.From the perspective of correlation,there is a greater degree of correlation between hepatic pathway and medicinal taste,which is closely related to bitterness and sweetness.3.Yi herb ironing method has good effect in alleviating the degree of gastrointestinal dysfunction,promoting anal exhaust,improving defecation,promoting the recovery of bowel sounds and other aspects of improving gastrointestinal function.It can effectively shorten the course of the disease,and has clinical significance in alleviating patients' pain.Its curative effect is obviously superior to the conventional treatment,and at the same time superior to the traditional Chinese medicine fennel hot ironing method,with the advantages of exact curative effect,safety and convenience. |
