| Objectives 1.To investigate the effects of pestle on clinical symptoms,neurological function and quality of life of the feet in patients with high-risk diabetic peripheral neuropathy.2.To evaluate the clinical efficacy and safety of pestle acupuncture in patients with high-risk diabetic peripheral neuropathy.Methods 1.A randomized,controlled trial was used in this study.132 inpatients with high-risk diabetic peripheral neuropathy who met the inclusion and exclusion criteria in the department of endocrinology,affiliated hospital of chengdu university of traditional Chinese medicine were randomly divided into control group and experimental group,with 66 patients in each group.2.Intervention methods: the control group received conventional treatment and nursing care;The experimental group on the basis of the control group increased needle pestle intervention,once a day,5 days a week,a total of 4 weeks of continuous intervention.3.Data collection:(1)general data: gender,age,body mass index,diabetes course,family history,smoking and alcohol history,foot ulcer history,fasting blood glucose,2-hour postprandial blood glucose,hemoglobin a1 c,etc.(2)the curative effect: after 4 weeks before the intervention and intervention of foot clinical symptom index(foot clinical symptoms integral table CSIT,Toronto grade TCSS,Michigan MINI),index of neuropathy(vibration feeling threshold VPT,section current threshold R-CPT),in patients with type 2 diabetes quality of life DMQLS and total to evaluate the clinical efficacy.Patients’ quality of life was followed up 3 and 6 months after intervention to evaluate the long-term effect.4.Statistical processing: this research adopts the SPSS21.0 statistical software for all data statistics,analysis and processing,and analysis between the two study groups intervention before and after the intervention,and followed up for 3 months,the indicators were followed up for 6 months after any differences,objective evaluation of the subjects’ baseline data before intervention and pestle on foot at risk for diabetes peripheral neuropathy of follow-up after the intervention,followed up for 3 months,6 months the effect and the security during the test.When the comparison between groups is carried out and the sample size is large(n≥50),it can be considered that the distribution of the sample mean is approximately normal(central limit theorem),and the normality test is unnecessary.When comparing between groups,if the two groups of data conform to normal distribution and homogeneity of variance,independent sample t test will be conducted for measurement data.If the variance is not equal,t ’test is performed.If the two groups of data do not conform to the normal distribution,the mann-whitney U test in the nonparametric test is adopted.The chi-square test was used for all counting data in this study.Results 1.A total of 126 cases were included in this study,including 62 cases in the control group and 64 cases in the experimental group.2.Before intervention,two groups of patients with general information: gender,age,body mass index,history,family history,alcohol,tobacco,duration of diabetes,a history of foot ulcers,fasting plasma glucose,postprandial 2 hours blood sugar,glycosylated hemoglobin,foot clinical symptom score:(CSIT,TCSS,MINI),indicators of neuropathy(VPT,R-CPT)and DMQLS are differences no statistical significance(P > 0.05),comparable.3.After intervention,there were statistically significant differences between the two groups in VPT,r-cpt left/right foot 250 Hz and 5Hz,CSIT,TCSS,MINI,DMQLS,total clinical efficacy and ulcer incidence(P < 0.05),and the experimental group was superior to the control group.The left/right foot 2000 Hz difference of r-cpt was not statistically significant(P > 0.05).4.The patients were followed up for 3 months and 6 months,and the difference in DMQLS between the two groups was statistically significant(P < 0.05).5.No foot ulcers or adverse reactions occurred in either group.Conclusion 1.Pestle needle therapy can improve the nerve function of patients’ feet,reduce the numbness,pain,weakness,cold and other clinical symptoms of patients’ limbs,so as to improve the quality of life of patients.2.Needle pestle therapy is a non-invasive operation,easy to master,high safety and easy to accept by patients,which is worthy of clinical promotion and application. |
