Objective To investigate the caries status of the first permanent molars in urban and rural children aged 10-12 in Chongqing and analyze the related factors which provide theoretical basis for the prevention and treatment of children" s caries and targeted suggestions for oral health education.Methods A multi-stage,stratified,cluster,and random sampling method was used to extract 5 057 children from 18 schools in 3 districts and 3 counties in Chongqing for oral health examination and questionnaire survey from March to May 2018.Results Thetotal caries rate ofthefirst permanent molar was 39.2%.The decayed-missing-filledtoothwas 0.84±1.20.The rate offillingteethwas 3.4%.The rate of pit and fissure sealing was 6.1%.The prevalence of the first permanent molars was statistically significant between men and women,between urban and rural areas,between only children and non-only children,between ethnic minorities and Han nationality(P<0.01).The gap between the pit and fissure sealing was statistically significant between urban and rural areas(P<0.001).The investigation of oral hygiene behavior shows that:57.9%of the children brush their teeth once in the morning and once in the evening;only 12.2%of the children brush their teeth for more than 3 minutes every time;37.3%of the children brush their teeth with the method of vertical brush along teeth seam;6.9%of the children use the dental floss everyday;only 10.4%of the children use the toothpaste with fluoride.The teeth brush condition of the children in the city and the condition of using toothpaste with fluoride are obviously better than the condition of the children in the countryside(p<0.05).The investigation of eating habits shows that:the children in the city often eat dessert and drink milk,of which the percentage is much higher than the children in the countryside(p<0.01);while the children in the countryside drink carbonated drinks and eat dessert more than once everyday,of which the frequency is more than the children in the city(p<0.01).The investigation on knowledge of oral health care shows that only 28.1%of the children have heard of the pit and fissure sealant;only 36.0%of the children believe that toothpaste with fluoride can protect the teeth effectively;only 28.5%of the children believe that the action of pit and fissure sealant can protect the teeth effectively.For other questions,the correct rate of answer is higher than 50%.The correct rate of answer by children in the city is much higher than the children in the city to the questions of“have you heard of pit and fissure sealant?”;“can pit and fissure sealant protect the teeth effectively?";“is it normal to have gum bleeding while brushing your teeth?","is brushing the teeth useless for the prevention of gum bleeding?"(p<0.01).The investigation of dealing with the gum bleeding after brushing the teeth shows that:61.3%of the children choose to rinse with clear water;only 11.5%of the children choose to ask a dentist for treatment;while the rate of children in the city choose a dentist for treatment(13.5%)is much higher than the children in the countryside(9.5%)(p<0.01).The results of multiple Logistic regression analysis showed that:there were many independent influencing factors for the occurrence of caries,including the frequency of brushing teeth every day,the frequency of eating desserts,the habit of eating dessert/drinking milk before going to bed,whether only children or not,parents'education background,etcConclusion The first permanent molars of children aged 10-12 in Chongqing have a high rate of caries,the rate of pit and fissure sealing and the rate of dental caries filling are low.The difference between urban and rural areas is large,children s oral health knowledge is scarce,and children have bad oral hygiene behaviors and eating habits.It is necessary to strengthen children' s oralhealth education,implement oral health preaching to schools,improve the oral health knowledge of children in school,and shorten urban-rural differences.
