Analysis Of The Efficacy Of Biological Lengthened Shank In The Treatment Of Elderly Patients With Intertrochanteric Fractures And Early Osteoarthritis Of The HIP Joint

Objective:to investigate the clinical effect and method of hip replacement in the treatment of elderly patients with intertrochanteric fracture of femur and early osteoarthritis of ipsilateral hip joint.And To explore the incidence of early osteoarthritis of hip joint in elderly patients.Methods: elderly patients with intertrochanteric fractures associated with ipsilateral early osteoarthritis of the hip who underwent joint replacement in a single center of the second affiliated hospital of chongqing medical university from July 2016 to December 2018 were statistically analyzed.Postoperative 3 months,12 months and 24 months follow-up on a regular basis and records,the average(18.67 + 5.97)months follow-up,the clinical evaluation,radiographic evaluation,VAS pain score and quality of life assessment(SF-36 scale)to evaluate curative effect,at the same time from the clinical choice make cartilage pathological femoral head load area,made of cartilage special dye solution turned red O to all good cartilage pathological dyeing parallel microscope analysis,according to the international society for arthritis(OARSI)pathological classificationstandard to assess cartilage arthritis severity.Results: 28 patients were followed up for an average of 18.67 5.97 months.Harris hip function score,VAS pain score and quality of life related assessment(sf-36 scale)were improved to some extent compared with those before surgery,and the difference was statistically significant.Intraoperative chondral pathological sections indicated 2patients with grade 0 OARSI,4 patients with grade 1 OARSI,8 patients with grade 2 OARSI,and 16 patients with grade 3 OARSI.The incidence of early osteoarthritis of the aged patients with intertrochanteric fracture combined with ipsilateral hip was 93.3%.After the surgery,all the patients had wound healing I/ a,no fractures around the prosthesis,no loosening and so on,resulting in revision,and good bone growth of the prosthesis,etc..One case had postoperative dislocation and good function after closed reduction,1 cases had deep vein thrombosis,1 case had pressure ulcer and3 cases had pulmonary infection.During the follow-up period,2 patients died and 2 patients were lost to follow-up.Conclusion: in this clinical study,according to the statistical analysis of Harris hip function score,cartilage pathological section results and quality of life score,the incidence rate of intertrochanteric fracture combined with early OA of the hip joint is higher in elderly patients(93.3%),and joint replacement has obvious effect in improving hip joint function,which is safe and effective.It is feasible to solve the problems ofearly osteoarthritis of ipsilateral hip joint and intertrochanteric fracture of femur simultaneously.Therefore,it is of great significance and excellent short-term clinical effect to alleviate pain,improve the function of the affected hip and improve the quality of life of patients.
